摘 要:目的:探讨大黄-黄芪不同剂量配比对慢性肾衰竭大鼠肠粘膜和肠道菌群移位的影响。方法:选取已建立慢性肾衰模型的大鼠分为模型组、贝那普利(5 mg/kg)组、大黄(1.5 g/kg)组、大黄-黄芪1:1(3.0 g/kg)组、大黄-黄芪1:2(4.5 g/kg)组、大黄-黄芪1:4(7.5 g/kg)组、大黄-黄芪1:6(10.5 g/kg)组。用显色基质鲎试剂终点显色法、生物素双抗体夹心酶联免疫吸附法、蛋白免疫印迹法和细菌平板计数法分别检测血浆内毒素、血清D-乳酸、回肠组织中occludin蛋白相对表达量及肠道菌群移位量。结果:(1)慢性肾衰模型组大鼠较假手术组大鼠内毒素、肠道细菌移位率显著升高,occludin蛋白相对表达量显著降低,D-乳酸水平显著升高;(2)与模型组比较,各给药组大鼠内毒素含量显著降低,大鼠脏器肠道菌群移位率除大黄-黄芪1:2组外,各给药组细菌移位率降低显著差异,occludin蛋白相对表达量显著升高,D-乳酸值有所降低;(3)与大黄组比较,大黄-黄芪不同剂量配比组中大黄-黄芪1:4组降低内毒素作用最为显著。结论:慢性肾衰会使大鼠肠粘膜发生损伤,肠通透性增加,血浆内毒素和肠道菌群发生移位;大黄-黄芪不同剂量配比可以改善慢性肾衰大鼠的肠粘膜损伤,降低肠粘膜通透性,使内毒素和肠道菌群移位得到抑制,其中以大黄-黄芪1:4组改善作用显著。Objective: To investigate effects of different ratios of Rhubarb and Astragale on intestinal mucosa and bacterial translocation in chronic renal failure rats. Method: The chronic renal failure rats were randomly divided into the model group, Benazepril( 5 mg/kg)group, Rhubarb group ( 1.5 g/kg) and different ratios of Rhubarb and Astragale ( 1 : 1, 1:2, 1:4, 1:6) (3.0,4.5,7.5, 10.5 g/kg) groups. The plasma en- dotoxine, serum D-lactate , the relative expression of ileum occludin proteins and gut bacteria translocation respectively were measured by the end point chromogenic reagent of TAL ( end-point method) , Biotin double antibody sandwich enzyme-linked immunosorbent method ( BA- ELISA), Western blot and plate counting of bacteria method. Results: ①Compared with the sham-operated group, levels of plasma endotox- ine and gut bacteria translocation in chronic renal failure rats increased ( p 〈 0.01 ) , the relative expressions of occludin proteins reduced ( p 〈 0.01 ) and serum D-lactate increased ( P = 0. 077 ) ; ②Compared with the model group, levels of endotoxine obviously reduced ( p 〈 0.01 ) , relative expressions of occludin proteins increased ( p 〈 0. 005 ) and the rate of gut bacteria translocation reduced ( P 〈 0.05 ) except in Rhubarb and Astragale (1:2) group, the serum D-lactate slightly reduced; ③Compared with the Rhubarb group, the level of endotoxine in Rhubarb and Astragale (1:4) group obviously reduced (P 〈0.05 ). Conclusion: ①Chronic renal failure can damage rats'intestinal muco- sa, increased intestinal permeability, plasma endotoxin and intestinal flora translocation; ②Different ratios of Rhubarb and Astragale can suppress the intestinal mucosa injury in rats with chronic renal failure , reduce intestinal permeability, endotoxin and suppress intestinal flora translocation, among them effects of Rhubarb and Astragale (1 :4) will be optimal.
