摘 要:目的观察加味牛蒡子汤(JWNBZT)对类风湿关节炎(RA)模型动物TNF-α转基因(TNF-Tg)小鼠关节炎症和关节周围淋巴回流功能的影响。方法将16只清洁级3月龄左右TNF-Tg小鼠按照体重随机分为TNF-Tg组和JWNBZT组,每组8只。另取10只同窝对照体重相似的野生型小鼠(WT)作为对照组,JWNBZT组每天以JWNBZT灌胃,对照组和TNF-Tg组每天以等量纯净水灌胃,治疗12周后小鼠足底皮下注射吲哚菁绿(ICG)测量小鼠淋巴管回流功能,并取踝关节制作石蜡切片进行苏木素-伊红(HE)染色观察炎性浸润和关节骨质破坏情况,抗酒石酸酸性磷酸酶(TRAP)染色观察破骨细胞数量。结果与对照组比较,TNF-Tg组小鼠淋巴管回流功能出现明显损害,Clearance和Pulse明显下降,淋巴管渗漏面积增多,踝关节内出现明显炎症浸润且骨质减少,距骨内破骨细胞增多(P<0.05);与TNF-Tg组比较,JWNBZT组小鼠淋巴管回流功能明显增强,Clearance和Pulse升高,淋巴管渗漏面积减少,踝关节内炎症浸润减轻且骨质增多,距骨内破骨细胞减少(P<0.05)。结论 JWNBZT能够减轻RA受累关节的炎症浸润和关节骨质破坏,并促进淋巴管回流功能,这可能是其治疗RA的机制。Objective To observe the effect of Jia-Wei-Niu-Bang-Zi-Tang(JWNBZT) on inflammatory damage at ankle joint tissue and draining function of peri-articular lymphatic in tumor necrosis factor-transgenic (TNF-Tg) mice, a widely used rheumatoid arthritis (RA) model. Methods Sixteen TNF-Tg mice, 3-month old, clean grade were divided into the TNF-Tg group and the JWNBZT-treated group based on body weight, 8 in each group. Ten wild type (WT) littermates with similar weight were used as a control group. Mice in the JWNBZT group were daily administered with JWNBZT by gastrogavage. Equal volume of pure water was daily administered to rats in the control group and the TNF-Tg group by gastrogavage. Twelve weeks later, all the mice were subcutaneously injected with indocyanine green (ICG)at the footpad under near-infrared laser (NIR)to assess the draining function of peri-articular lymphatic vessels. HE staining was performed on ankle joints to observe inflammatory damage and infiltration, and TRAP staining was performed to observe the amount of osteoclasts. Results Compared with the control group, the significant defect of lymphatic draining function was observed in the TNF-Tg group with obviously decreased Clearance and Pulse as well as enlarged area of lymphatic leakage. In addition,the TNF-Tg mice showed severe inflammatory infiltration and loss of bone mass at ankle, with lageramount of osteoclasts in talus (P 〈0.05). As compared with those in the TNF-Tg group, JWNBZT signifi- cantly enhanced the lymphatic draining function, increased Clearance and Pulse, reduced lymphatic leakage area and attenuated the inflammatory infiltration, increased bone mass, and decreased the number of osteoclasts in talus (P 〈0.05). Conclusion JWNBZT could alleviate inflammation infiltration and bone destruction of involved joints in RA patients, and promote lymphatic draining function, which might be one of potential mechanisms for treating RA.
