摘　　要：目的:了解厦市第二医院(以下简称"我院")中药不合理应用情况及原因,为临床合理用药提供依据。方法:提取我院2016年4—9月的处方共20 669张,对其合理性进行回顾性评价与分析。结果:20 669张处方中,不合理处方201张(占0.97%),排序居前3位的科室分别为便民科、呼吸内科和重症加强护理病房;不合理处方类型主要包括剂量过大或过小、处方剂数过多、书写不规范、脚注错误或不写等。结论:我院中药处方较为合理,但也存在一定的不合理性,临床应加强对处方的管理,促进临床合理用药。OBJECTIVE：To investigate the irrational drug application of TCM in the Second Hospital of Xiamen （hereinafter referred to as ＂our hospital＂ ） and analyze the reasons, so as to provide references for clinical rational drug application. METHODS： 20669 prescriptions in our hospital from Apr. 2016 to Sep. 2016 were extracted and retrospectively analyzed on their rationalities. RESULTS： Of 20669 prescriptions, there were 201 irrational prescriptions （0.97 % ）, the top three clinical departments were respectively department of convenience, department of respiratory medicine and ICU; Types of irrational prescriptions mainly included overdose or too small dose, over- prescription, nonstandard writing, incorrect footnote or no footnote, etc. CONCLUSION： There are still some irrational prescriptions in our hospital which should be administrated strictly, so as to improve clinical rational drug application.
