摘  要:目的探讨急性肾损伤肾脏超声造影血流灌注的变化及其临床价值。方法选取24例入院时符合急性肾损伤(AKI)诊断标准的住院患者作为研究对象,应用声诺维团注及低机械指数技术对其肾脏进行超声造影检查,Q-LAB软件绘制时间-强度曲线并获得各参数。在超声造影当日,符合AKI诊断标准者17例34枚肾脏作为病例组,未达到AKI诊断标准者7例共13枚肾脏作为对照组。对两组间肾脏超声造影模式及参数进行分析和比较。结果与对照组比较,病例组肾脏超声造影参数PI减低,其余参数均无统计学差异。结论超声造影可以动态观察肾脏的灌注过程。肾皮质造影峰值强度能较灵敏地反映出AKI患者肾脏血流灌注的变化。Objective To investigate the perfusion changes of acute kidney injury renal with contrast-enhanced ultrasound and evaluate its clinical value. Methods Twenty-four patients with acute kidney injury were enrolled in the study. Contrast-enhanced ultrasound examinations with low mechanical index technology and SonoVue bolus injection were performed on all patients' kidneys. Time intensity curves were generated by Q-LAB, and a series of parameters of renal blood perfusion were obtained. Seventeen patients who met the diagnostic criteria of AKI were as case group, with a total of thirty-four kidneys. Seven patients who did not meet the diagnostic criteria of AKI were as control group, with a total of 13 kidneys. The parameters and mode of enhancement were compared and analyzed between the two groups. Results Compared with the control group, PI (peak intensity) in the case group decreased, and there was no statistical difference in the other parameters between two groups. Conclusions Contrast-enhanced ultrasound could be used to observe the perfusion of kidney dynamically. PI of the renal cortex could reflect the changes of renal perfusion in patients with AKI sensitively.
