快速康复在老年股骨粗隆间骨折内固定失效后人工全髋关节置换围手术期应用  

Perioperative Application of Fast-Track Surgery Care to Total Hip Arthroplasty after Internal Fixation Failure of Elderly Patients with Intertrochanteric Fracture

摘　　要：目的研究快速康复理念在老年股骨粗隆间骨折内固定失效后人工全髋关节置换围手术期的应用效果。方法选择2014年2月至2015年4月股骨粗隆间骨折内固定失效行人工全髋置换治疗的老年患者32例,年龄(65.50±1.64)岁。术后应用快速康复理念进行护理,随访12个月。结合术前、术后髋关节功能Harris评分和术后并发症等情况进行分析。结果 32例股骨粗隆间骨折内固定失效后人工全髋关节置换手术治疗均获得成功,手术时间70~100 min,术中、术后无骨折;术后无伤口感染、肺炎、褥疮、下肢深静脉血栓等并发症;住院及随访期间未出现感染、假体脱位、松动、断裂和骨折等并发症,术后Harris评分达到良好以上者30例;2例随访期间资料部分丢失。结论快速康复理念在老年股骨粗隆间骨折内固定失效后人工全髋关节置换围手术期的应用,能明显提高老年患者住院质量,减少并发症,加快髋关节功能恢复速度。Objective To study application effect of fast-track surgery care for total hip arthroplasty after internal fixation failure of elderly patients with intertrochanteric fracture during the perioperative period.Methods The 32 elderly patients with internal fixation failure for total hip arthroplasty,（ 65. 50 ± 1. 64）years old with intertrochanteric fracture,were cared by fast-track surgery care. Follow-up of 12 months,There were we analyzed the effect by Harris score of preoperative and postoperative hip function,postoperative complications and so on. Results The 32 patients surgical treatment were successful（ operation time between 70 and 100 min） without fracture in intraoperative and postoperative. No complications happened including wound infection,pneumonia,bed sores,deep vein thrombosis of lower limbs,and no infection,prosthesis dislocation,loose,fracture,fracture and other complications appeared during hospitalization and follow-up. 30 cases postoperative Harris score reached good level. 2 cases data were lost during the follow-up. Conclusion Application of fast-track surgery care for total hip arthroplasty after internal fixation failure to elderly patients with intertrochanteric fracture could significantly improve their quality of hospitalization,reduce complications and speed up the recovery of hip joint function.
