ICU肺部感染者应用无创机械通气的效果

摘　　要：目的 探讨无创机械通气治疗ICU肺部感染患者的临床疗效.方法 分析80例重症肺炎患者的临床资料,根据呼吸支持技术分为无创机械通气组（NIPPV组）和有创机械通气组（IV组）,每组40例,比较两组患者的心率、血压、血气分析指标、机械通气时间、ICU住院时间及生存率.结果 两组治疗后心率、血压、血气分析指标均有明显改善,但组间比较差异未见统计学意义（P〉0.05）;NIPPV组呼吸机辅助呼吸的时间、ICU住院时间均短于IV组,差异有统计学意义（P〈0.05）;NIPPV组生存率为72.5%,高于IV组的60.0%,差异有统计学意义（P〈0.05）.结论 采用无创机械通气治疗ICU肺部感染患者可获得与有创通气相似的效果,同时可以减少气管插管率,缩短机械通气时间,提高总体生存率.Objective To investigate the clinical efficacy of noninvasive mechanical ventilation on patients with pulmonary infection in ICU.Methods The clinical data of 80 patients with severe pneumonia were analyzed,and the patients were divided into noninvasive mechanical ventilation group （NIPPV group,40 cases） and invasive mechanical ventilation group （IV group,40 cases） according to respiratory support technology.The heart rate,blood pressure,blood gas analysis index and duration of mechanical ventilation,ICU length of stay and survival rates were compared between the two groups.Results The heart rate,blood pressure,blood gas analysis indexes improved significantly after treatment in two groups,but the differences between the two groups were not significant （P〉0.05）.The duration of mechanical ventilation,ICU length of stay in NIPPV group were less than those in IV group,the differences were significant （P〈0.05）.The survival rate of NIPPV group was 72.5%,which was higher than that of IV group （60.0%）,the difference was significant （P〈0.05）.Conclusions The patients with pulmonary infection in ICU treated by noninvasive mechanical ventilation can acquire the similar therapeutic effect of invasive ventilation,and decrease the duration of invasive MV,reduce the re-intubated rate and improve the overall survival rate.
