A-JUST与TVT-O治疗女性压力性尿失禁疗效分析

摘　　要：目的 对比单切口可调节A-JUST吊带与经闭孔无张力尿道中段悬吊TVT-O吊带治疗女性压力性尿失禁的临床疗效.方法 回顾性分析2013年1月至2014年12月由同一手术医师完成的A-JUST和TVT-O两种尿道中段悬吊术式患者的临床资料.评估两组患者的一般资料、围术期参数、术后生活质量、治愈率及并发症发生率.结果 共105例符合标准并完成随访患者入选,两组术前一般资料比较差异未见统计学意义（P〉0.05）,具有可比性.两组手术时间、术中出血量、术后留置尿管时间比较差异未见统计学意义（P〉0.05）;A-JUST组住院天数、腹股沟疼痛率少于TVT-O组,差异有统计学意义（P〈0.05）.两组术后随访尿失禁生活质量问卷（I-QOL）评分较术前提高,差异有统计学意义（P〈0.05）;两组间I-QOL评分比较差异未见统计学意义（P〉0.05）.A-JUST组主观治愈率为88.7%,客观治愈率为88.7%;TVT-O组主观治愈率为92.2%,客观治愈率为86.5%,两组比较差异未见统计学意义（P〉0.05）.A-JUST组与TVT-O组远期并发症发生率比较差异未见统计学意义（P〉0.05）.结论 A-JUST与TVT-O均有效治疗女性压力性尿失禁,但A-JUST具有住院时间短、术后腹股沟疼痛率低等优势.Objective To compare the clinical effects of adjustable single-incision sling（A-JUST） and transobturator vaginal tape （TVT-O） on female stress urinary incontinence （SUI）.Methods The clinical data of female SUI patients who underwent A-JUST or TVT-O procedure by one same doctor from January.2013 to December.2014 were retrospectively analyzed.The preoperative general data,peri-operative data,postoperation quality of life,cure rate and complications were observed.Results A total of 105 patients who met the standards and completed survey were enrolled.There were no significant differences in preoperative general data between the two groups （P〉0.05）.The two groups are comparable.There were no significant differences in operative time,intraoperative hemorrhage or the preserved time of installing catheter after operation between the two groups（P〉0.05）.Compared with the TVT-O group,the hospital days and incidences of groin pain in the A-JUST group were lower（P〈0.05）.Both A-JUST group and TVT-O group had significant improvement in I-QOL（P〈0.05）,but there was no significant difference in the average improvement of I-QOL between the two groups （P〉0.05）.The subjective cure rate and objective cure rate in A-JUST groups was 88.7% and 88.7%,and those in TVT-O group was 92.2% and 86.5%,there was no significant difference（P〉0.05）.There was no significant difference in the incidence of long-term complications between the two groups（P〉0.05）.Conclusions Both A-JUST procedure and TVT-O procedure are effective and reliable in treatment of female SUI,A-JUST procedure has advantages of lower hospital days and incidence of groin pain.
