甲状腺手术围手术期预防性使用抗菌药物与术后伤口感染的关系研究

摘　　要：目的探讨甲状腺手术围术期预防性使用抗菌药物与术后伤口感染的关系。方法选取2015年9月-2016年10月接受治疗的甲状腺手术患者80例,采用随机数表法将其分为观察组40例,对照组40例;对照组患者未给予抗菌药物治疗,观察组患者则给予抗菌药物治疗,观察比较两组患者治疗后的住院时间、药品费用、手术时间、切口甲级愈合率、并发症发生率以及术后不同时间点患者的体温变化。结果两组患者手术时间比较差异无统计学意义,观察组患者药品费用高于对照组患者,差异有统计学意义(P<0.05),观察组患者住院时间长于对照组患者,差异有统计学意义(P<0.05);两组患者治疗后切口甲级愈合率以及术后咳嗽咳痰、肺部感染、咽痛等并发症的发生率与对照组患者比较,差异无统计学意义;两组患者术后各时间段体温比较,差异无统计学意义。结论对甲状腺手术患者围术期不应用抗菌药物不会影响患者的预后,同时能够缩短患者的住院时间,降低医疗费用,有效地促进患者术后恢复,可行性高,值得临床推广使用。OBJECTIVE To investigate the relationship between perioperative prophylactic use of antibiotics and postoperative wound infection in patients with thyroid surgery.METHODS A total of 80 patients with thyroid surgery from Sep.2015 to Oct.2016 in our hospital were selected,and were divided into two groups using random number table method,including observation group of 40 cases and control group of 40 cases.Patients in control group were not treated with antibiotics,patients in observation group were given antibiotic treatment,and the hospitalization time,drug costs,operation time,incision healing rate,complication rate and postoperative temperature changes at different time points between the two groups after treatment were observed and compared.RESULTS There was no significant difference in the operation time between the two groups.The drug costs of observation group was significantly higher than that of control group,and the hospitalization time of observation group was significantly long er than that of control group（P〈0.05）,Incision healing rate,postoperative cough and expectoration,pulmonary infection,cough and sore throat and other complications after treatment of the two groups had no significant difference,and the postoperative temperatures at different time points between the two groups had no significant difference.CONCLUSION Perioperative application of antibiotics will not affect the prognosis of patients for patients with thyroid surgery,it can shorten the hospitalization time,reduce medical costs,and effectively promote the postoperative recovery of patients,which has high feasibility and is worthy of clinical use.
