磷酸奥司他韦联合炎琥宁对儿童流感病毒感染患儿免疫功能及预后的影响分析

摘　　要：目的探究磷酸奥司他韦联合炎琥宁对儿童流感病毒感染患儿免疫功能及预后的影响分析。方法选择医院2015年7月-2016年2月收治的流感病毒感染患儿61例,采用随机、平行、双盲的方法分成联合组31例和对照组30例,联合组接受磷酸奥司他韦联合炎琥宁治疗,对照组仅接受磷酸奥司他韦治疗,观察两组患儿免疫功能改善及预后情况。结果经不同治疗后,联合组患儿发热消退、乏力缓解、肺部影像学积分及病毒核酸转阴时间分别为(7.45±1.49)d、(9.01±1.69)d、(1.64±0.59)分及(6.52±1.51)d,均低于对照组(9.17±2.31)d、(11.67±2.18)d、(2.71±0.92)分及(9.49±2.63)d,差异有统计学意义(P<0.05);联合组CD_4^+T细胞、CD_8^+T细胞、CD_3^+T细胞、NK细胞比例均明显高于对照组,差异有统计学意义(P<0.05);在预后方面,联合组治愈率及显效率分别为41.94%、51.61%,总有效率为93.55%,与对照组23.33%、50.00%及73.33%比较,差异有统计学意义(P<0.05)。结论磷酸奥司他韦联合炎琥宁可以更好地提高流感病毒感染患儿的淋巴细胞亚群比例,提高免疫功能,改善临床症状及预后转归,值得推荐使用。OBJECTIVE To explore the effect of oseltamivir phosphate combined with Yanhuning on immune function and prognosis in children with influenza virus infection.METHODS A total of 61 cases of children with influenza virus infection from Jul.2015 to Feb.2016 were selected,and were divided into the combined group（n=31） and control group（n=30） with randomized,parallel,double-blind method.Patients in combined group were received oseltamivir phosphate combined with Yanhuning,and in control group were received only oseltamivir phosphate.The improvement of immune function and the prognosis of the two groups were observed.RESULTS After the treatment,the fever subsided,remission of fatigue,pulmonary imaging score and the time of virus nucleic acid negative were （7.45±1.49） d,（9.01±1.69）d,（1.64±0.59） points and （6.52±1.51） d,which were significantly lower than （9.17±2.31）d,（11.67±2.18）d,（2.71±0.92）points and （9.49±2.63）d of control group （P〈0.05）.The percentage of CD4 ＋T cells,CD8 ＋T cells,CD3＋T cells and NK cells in combined group were significantly higher than those in control group （P〈0.05）.In the prognosis,the cure rate and efficiency of combined group were 41.94% and 51.61 %,and the overall effective rate was 93.55%,and the difference was significant compared to 23.33%,50.00% and 73.33% of control group （P〈0.05）.CONCLUSION Oseltamivir phosphate combined with Yanhuning can improve the proportion of lymphocyte subsets in children with influenza virus infection,and improve immune function,clinical symptoms and prognosis,so it is recommended to use.
