快速康复外科理念结合中医特色护理在大肠癌围手术期的应用

摘　　要：目的探讨快速康复外科理念结合中医特色护理在大肠癌围手术期的临床应用效果,旨在提供一种新的中西医结合护理模式。方法选择2014年6月—2016年8月中山市中医院肛肠科大肠癌手术患者120例,通过前瞻性随机对照研究的实验方法,设60例为对照组,采用传统常规护理方案;60例为试验组,采用快速康复外科理念结合中医特色护理方案,即对患者加强健康宣教,术前进一定量的流食,取消清洁灌肠,术中注重保温,术后镇痛、抗炎补液、早期肠内营养、指导患者早期活动,并配合中医耳穴压豆、穴位按压、吴茱萸粉外敷神阙穴等特色护理。结果试验组60例大肠癌患者术后肠鸣音恢复时间、首次排气时间、首次排便时间、下地时间、住院天数、并发症发生率与传统护理组比较,差异均有统计学意义(P<0.05)。结论快速康复外科理念结合中医特色护理方案应用于大肠癌围手术期患者可加速术后康复、减少住院时间和费用,这种新的中西医结合护理模式值得在临床上推广应用。Objective： To explore the clinical application effect of the concept of rapid rehabilitation surgery combined with traditional Chinese medicine nursing in the perioperative period of eolorectal cancer, and to provide a new nursing mode for Chinese traditional and Western medicine. Methods： 120 cases of eolorectal cancer patients at Zhongshan Hospital of Tradi- tional Chinese Medicine from June, 2014 to August, 2016 were randomly divided into 60 patients in the control group treated the routine nursing care; 60 eases as experimental group treated by the concept of rapid rehabilitation surgery combined with TCM characteristic nursing program, including strengthening the health education of patients, drinking a certain amount of liquid before operation, canceling cleaning enema, paying attention to insulation during operation, postoperative analgesia, antiinflammatory, rehydration, early enteral nutrition, guidance of early activity in patients in combination with traditional Chinese medicine of auricular plaster therapy and acupressure, Evodia powder Shenque and other special care. Results： In the comparison of postoperative recovery time of bowel sounds, the first exhaust time, first defecation time, ambulation time, hospitalization time, complication rate and traditional nursing group, the differences between the two groups were statistically significant （ P 〈 0.05 ）. Conclusion： The concept of rapid rehabilitation surgery combined with TCM nursing care applied to coloreetal cancer patients in the perioperative period can accelerate postoperative rehabilitation, reduce hospitalization time and cost, which is worth the clinical application.
