摘　　要：本文主要对延续护理的研究现状及进展进行综述,重点从延续护理的概念和发展历程、脑卒中延续护理的重要性及干预现状等方面进行探讨,为今后延续护理模式的实施提供科学依据.This article mainly summarized the present situation and the development of continuous nursing,which focused on the concept and development of continuous nursing,and the importance and the present situation of the continuous nursing,so as to provide scientific basis for the implementation of continuous nursing mode in the future.
