摘 要:目的研究老年颅脑损伤急性期患者格拉斯哥昏迷评分(GCS)评分与血清中超敏C反应蛋白(hsCRP)、基质金属蛋白质-2(MMP-2)和基质金属蛋白质-9(MMP-9)的关系,为临床诊疗提供依据。方法选取2014年1月至2015年1月我院收治的老年颅脑损伤急性期患者150例,根据GCS评分将患者分为轻度组(57例)、中度组(51例)和重度组(42例),另选取同期健康体检者50例作为对照组,检测各组受检者的hs-CRP、MMP-2和MMP-9水平,应用Spearman秩相关性分析GCS评分与hs-CRP、MMP-2和MMP-9的关系。结果对照组受检者的hs-CRP、MMP-2和MMP-9水平分别为(2.09±0.62)ng/mL、(15.23±0.86)ng/mL、(13.09±1.59)ng/mL,均明显低于轻度组的(7.09±1.32)ng/mL、(87.08±5.34)ng/mL、(70.53±8.53)ng/mL以及中度组的(18.34±0.87)ng/mL、(103.64±6.62)ng/mL、(108.71±6.08)ng/mL和重度组的(28.13±0.63)ng/mL、(189.42±9.03)ng/mL、(189.42±2.64)ng/mL,轻度组hs-CRP、MMP-2和MMP-9水平均明显低于中度组和重度组,中度组hs-CRP、MMP-2和MMP-9水平明显低于重度组,差异均具有统计学意义(P<0.05);Spearman秩相关性分析显示,GCS评分与hs-CRP、MMP-2和MMP-9均呈负相关(P<0.05)。结论老年颅脑损伤急性期患者的hs-CRP、MMP-2和MMP-9水平均会升高,且GCS评分与hs-CRP、MMP-2和MMP-9水平呈负相关关系,临床上可通过测定血清hs-CRP、MMP-2和MMP-9水平来反映颅脑损伤程度。Objective To study the relationship of Glasgow Coma Scale (GCS) score and serum high sensitive C reactive protein (hs-CRP), matrix metal protein-2 (MMP-2) and matrix metal protein-9 (MMP-9) in elderly patients with acute craniocerebral injury, and to provide basis for clinical diagnosis and treatment. Methods A total of 150 cas-es of elderly patients with acute craniocerebral injury in our hospital from January 2014 to January 2015 were selected. They were divided into mild group (57 cases), moderate group (51 cases) and severe group (42 cases) according to the GCS score. At the same time, 50 healthy physical examination were selected as the control group. The levels of hs-CRP, MMP-2 and MMP-9 were detected in each group. Relationship between GCS score and hs-CRP, MMP-2 and MMP-9 were analyzed by Spearman rank correlation. Results The levels of hs-CRP, MMP-2, MMP-9 were respec-tively (2.09 ± 0.62) ng/mL, (15.23 ± 0.86) ng/mL and (13.09 ± 1.59) ng/mL in the control group, which were significantly lower than (7.09 ± 1.32) ng/mL, (87.08 ± 5.34) ng/mL, (70.53 ± 8.53) ng/mL in the mild group, (18.34 ± 0.87) ng/mL, (103.64±6.62) ng/mL, (108.71±6.08) ng/mL in moderate group and (28.13±0.63) ng/mL, (189.42±9.03) ng/mL, (189.42± 2.64) ng/mL in severe group. The levels of hs-CRP, MMP-2 and MMP-9 in mild group were significantly lower than those in the moderate group and the severe group, and the levels of hs-CRP, MMP-2 and MMP-9 in the moderate group were significantly lower than those in the severe group, P〈0.05. Spearman rank correlation correlation analysis showed that GCS score was negatively correlated with hs-CRP, MMP-2 and MMP-9 (P〈0.05). Conclusion The hs-CRP, MMP-2 and MMP-9 levels in elderly patients with acute craniocerebral injury are increased, and the GCS scores are negatively correlated with hs-CRP, MMP-2 and MMP-9 levels.
