无创双水平正压通气治疗新生儿呼吸衰竭的临床效果

摘　　要：目的观察分析无创双水平正压通气治疗新生儿呼吸衰竭的临床效果。方法 80例呼吸衰竭新生儿作为研究对象,根据数字随机分配的原则将其分为观察组与对照组,各40例。对照组患儿给予常规头罩给氧治疗,观察组患儿给予无创双水平正压通气治疗。观察对比两组患儿的临床治疗效果。结果治疗1 h后观察组动脉血氧分压(PaO_2)为(78.3±20.0)mm Hg(1 mm Hg=0.133 kPa)、动脉血二氧化碳分压(PaCO_2)为(36.5±11.2)mm Hg,均优于对照组的(53.4±11.5)、(42.4±10.9)mm Hg,差异均具有统计学意义(P<0.05)。观察组患儿呼吸功能恢复正常所需时间、平均住院时间分别为(54.2±6.6)h、(6.1±1.7)d,均短于对照组的(69.0±13.8)h、(9.6±2.6)d,差异均具有统计学意义(P<0.05)。结论无创双水平正压通气治疗新生儿呼吸衰竭具有良好的临床疗效。Objective To observe and analyze the clinical effect of noninvasive double level positive pressure ventilation in the treatment of neonatal respiratory failure. Methods A total of 80 newborns with respiratory failure as study subjects were divided by principle of digital randomly assigned into observation group and control group, with 40 cases in each group. The control group received conventional hood for oxygen therapy, and the observation group received noninvasive double level positive pressure ventilation for treatment. Clinical therapeutic effect difference was ob±served and compared in two groups. Results After 1 h of treatment, the observation group had arterial partial pressure of oxygen （PaO2） as （78.3 ± 20.0） mm Hg （1 mm Hg=0.133 kPa）, arterial partial pressure of carbon dioxide （PaCO2） as （36.5 ± 11.2） mm Hg, which were all better than （53.4 ± 11.5） and （42.4 ± 10.9） mm Hg in the control group, and their difference had statistical significance （P〈0.05）. The observation group had time needed for respiratory function back to normal and average hospital stay time respectively as （54.2 ± 6.6） h and （6.1 ± 1.7） d, which were all shorter than （69.0 ± 13.8） h and （9.6 ± 2.6） d in the control group. Their difference had statistical significance （P〈0.05）. Conclusion Noninvasive double level positive pressure ventilation shows excellent clinical curative effect in treating neonatal respiratory failure.
