血液灌流联合血液透析治疗急性百草枯中毒疗效观察

摘　　要：目的分析目前治疗急性百草枯中毒方法。方法 74例急性百草枯中毒患者,根据治疗方法不同分为内科治疗组(34例)、血液灌流治疗组(20例)和血液灌流+血液透析治疗组(20例)。内科治疗组给予洗胃、维持水电解质平衡、保护内脏功能等对症支持治疗,血液灌流治疗组在内科治疗的基础上加用血液灌流治疗,血液灌流+血液透析治疗组在内科治疗的基础上加用血液灌流及血液透析联合治疗。比较三组患者治疗效果。结果血液灌流+血液透析治疗组死亡率为20.00%,低于血液灌流治疗组的50.00%(P<0.05),血液灌流+血液透析治疗组及血液灌流治疗组死亡率低于常规内科治疗组的76.47%,差异有统计学意义(P<0.05)。三组患者12 h内开始治疗的患者其预后均明显优于12 h后接受治疗的患者,差异有统计学意义(P<0.05)。结论对于急性百草枯中毒患者血液灌流联合血液透析治疗能够提高治疗效果,改善预后,提升患者存活率。Objective To analyze the method for the treatment of paraquat poisoning at present. Methods A total of 74 acute paraquat poisoning patients were divided by different treatment methods into internal medicine treatment group （34 cases）, blood perfusion treatment group （20 cases） and blood perfusion ＋ hematodialysis treatment group （20 cases）. Internal medicine treatment group received symptomatic and supportive treatment of Gastric lavage, maintaining water and electrolyte balance, protecting liver function, blood perfusion treatment group also received blood perfusion treatment, and blood perfusion ＋ hematodialysis treatment group also received blood perfusion and hematodialysis for combined treatment. Treatment effect was compared in three groups. Results Blood perfusion ＋ hematodialysis treatment group had lower death rate as 20.00% than 50.00% in blood perfusion treatment group （P〈0.05）, and blood perfusion ＋ hematodialysis treatment group had lower death rate than 76.47% in internal medicine treatment group. Their difference had statistical significance （P〈0.05）. Patients treated within 12 h had obviously better prognosis than patients treated after 12 h in three groups, and the difference had statistical significance （P〈0.05）. Conclusion Combination of blood perfusion and hematodialysis can improve treatment effect of acute paraquat poisoning patients, improve their prognosis and increase their survival rate.
