摘 要:目的探究腺性膀胱炎的性质以及恶变的可能性,分析临床分型的合理性。方法结合膀胱镜下具体情况,将疾病分为低危型和高危型。取新鲜组织50例(高危24例,低危26例),另取14例无病变对照品,使用流式细胞术检测DNA水平。取76例蜡块组织(低危40例,高危36例)。使用免疫组化法探究增殖细胞核抗原(PCNA)、人体抑癌基因(p53)、ras基因蛋白p2(p21ras)、bcl-2及Rb的表达情况。结果 p21ras高危组p53以及PCNA阳性表达比低危组高,差异均有统计学意义(均P<0.05)。Rb、bcl-2表达在高危和低危组中差异无统计学意义(P>0.05)。无病变者的DI为0.96~1.06(0.99±0.04),低危型为0.95~1.05(1.02±0.06),高危型为0.97~1.19(1.06±0.08),差异无统计学意义(P>0.05)。正常和低危型均为二倍体,高危者中有6例存在异倍体。结论高危以及低危腺性膀胱炎均为良性病变,与低危型相比,高危型癌变几率更大。因此将该疾病加以分级具备合理性。p53基因在高危型疾病恶性病变中起到一定作用。Objective To explore the nature of the gland sex cystitis and potential malignant transformation, analyze the clinical classification rationality. Methods Combined with the specific situation under cystoscope, the disease was divided into low risk and high risk, fresh groups 50 cases, 24 cases of high risk, low risk of 26 cases, the other 14 cases with no pathological changes reference substance. Using immunohistoehemical method to explore the mutant, PCNA, p53, p21 ras, BCL - 2, and the expression of Rb. Results p53 and PCNA in the high - risk group of positive expression was significantly higher than low risk group, data exist significant differences between groups, P 〈0. 05. Rb p21 ras and bcl- 2 indicators in high -risk and low -risk groups there was no significant difference in the expression, no pathological chan- gesofDI - 1.06 -0.96(0.99 _+0.04). Low -risk type 0.95 -1.05 (1.02 _+0.06), high -risk type 0.97-1.19 ( 1.06 _+ 0. 08 ). Two DI value did not exist significant differences in the types of disease ( P 〉 0. 05 ). Both normal and low risk types were diploid, there were 6 cases in high risk people heteroploid. Conclusion Genotyping of high risk and low risk sexual cystitis gland are benign lesions, compared with low -risk type, high -risk type cancer risk is bigger. So will the disease classification correctness, p53 gene in malignant lesions in high -risk type disease have played a role.
