摘  要:目的:探讨补体C1q/肿瘤坏死因子相关蛋白1(CTRP1)与急性ST段抬高型心肌梗死(STEMI)患者病变严重程度的相关性。方法:选取2010-09-2015-09在我院首次明确诊断为STEMI且行急诊经皮冠状动脉介入治疗(PCI)的患者246例,根据CTRP1水平分为高CTRP1组和低CTRP1组。结果:低CTRP1组患者Gensini积分比高CTRP1组患者高,差异具有统计学意义(P<0.05)。CTRP1水平随冠脉病变支数和植入支架的数量的增加而降低,且差异均具有统计学意义(P<0.05)。CTRP1与Gensini积分(r=-0.243,P=0.007)、Hs-CRP(r=-0.313,P=0.012)呈负相关,与LVEF(r=0.172,P=0.035)呈正相关。逐步多元线性回归显示:年龄、TC、LDL-C、Hs-CRP、LVEF、左主干病变和冠脉病变支数为Gensini积分的独立影响因素。结论:CTRP1与急性STEMI患者冠脉病变严重程度密切相关,能够作为急性STEMI患者病情严重程度的预测因子。Objective:To study the relation between CTRP1 and acute ST segment elevation infarction in pa- tients. Method:From September 2010 to September 2015,246 patients with PCI were diagnosed as STEMI in the first time,and were divided into high CTRP1 group and low CTRP1 group according to the CTRP1 level. Result: Low CTRP1 group patients with Gensini score higher than the CTRP1 group,the difference was statistically sig- nificant(P〈0.05). The level of CTRP1 decreased with the increase of the number of coronary lesions and the number of implanted stents,and the difference was statistically significant(P〈0.05). CTRPlwas negatinely relat- ed with Gensini (r = - 0. 243, P = 0. 007) and Hs-CRP (r = - 0.313, P = 0. 012 ), CTRP1 was positively related with the LVEF (r=0. 172 ,P〈0. 035). Stepwise multiple linear regression showed that age,TC,LDL-C, Hs-CRP, LVEF,left main coronary artery disease and coronary artery lesion were independent factors of Gensini score. Con- elusion:CTRP1 is related with acute ST segment elevation infarction in patients,and can be used as a predictor of the severity of the disease in patients with acute STEMI.
