扶正固本Ⅰ号方对肺脾两虚型晚期非小细胞肺癌患者免疫功能及化疗副反应影响的临床观察

摘　　要：目的观察扶正固本Ⅰ号方对肺脾两虚型晚期非小细胞肺癌(NSCLC)患者免疫功能及化疗副反应的影响。方法将60例NSCLC患者随机分为治疗组与对照组,每组30例。对照组予常规化疗方案,治疗组在对照组化疗方案基础上加服扶正固本Ⅰ号方。对照组疗程12周,治疗组疗程13周。观察化疗所致毒副反应情况,比较血清中T细胞亚群(CD3^+、CD4^+、CD8^+等)及免疫细胞因子(IFN-γ、IL-10)水平的变化情况。结果 (1)组间治疗后比较,CD3^+、CD4^+、CD4^+/CD8^+、CD4^+CD25^+Treg水平差异有统计学意义,治疗组T细胞亚群水平改善明显优于对照组(P<0.05)。(2)组间治疗后比较,治疗较对照组IFN-γ水平明显升高,但IL-10水平明显下降(P<0.05)。(3)疗程结束后组间比较,治疗组恶心呕吐、白细胞减少、肝损伤分级较对照组明显改善(P<0.05),而肾损伤分级差异无统计学意义(P>0.05)。同时,组间各毒副反应发生率(恶心呕吐、白细胞减少、肝损伤)比较,治疗组均明显低于对照组(P<0.05)。结论扶正固本Ⅰ号方能够显著提高肺脾两虚型晚期NSCLC患者的免疫功能,减轻化疗毒副反应。Objective To observe the effects of Fuzheng Guben Recipe I on the immune function and side reactions of chemotherapy in patients of advanced non small cell lung cancer（NSCLC） with syndrome of lung and spleen deficiency. Methods 60 patients with NSCLC were randomly divided into the treatment group and control group ,30 cases in each group. The control group was treated with conventional chemotherapy, and the treatment group was treated with Fuzheng Guben Recipe I based on the treatment for the control group. The treatment course was 12 weeks in the control group and 13 weeks in the treatment group. The toxic and side reactions induced by chemotherapy were observed, the changes of serum levels of T cell subsets （ CD3 ~ , CD4 ~ , CD8 ~ ） and immune cytokines （ IFN-y, IL- 10） were compared. Results （~）After treatment, there were statistically significant differences on the levels of CD3 ＋ , CD4 ＊ , CD4 ~/CD8 ~ , CIM ~ CD25 ~ Treg between the two groups, the improvement on the levels of T cell subsets in the treatment group was obviously better than that in the control group （ P 〈 0.05 ）. （~）Compared with the control group after treatment, the IFN-y level was significantly increased in the treatment group, while the IL- 10 level was significantly decreased（P 〈 0.05 ）. （~）Compared with the control group after treatment, the reactions of nausea and vomiting, leukopenia and liver injury were significantly improved in the treatment group （ P 〈 0.05 ） , while there was no statistically significant difference on the grade of kidney injury between the two groups （ P 〉 0.05 ）. The incidence rates of toxic and side reactions including nausea and vomiting, leukopenia and liver injury in the treatment group were all obviously lower than those in the control group （ P 〈0. 05 ）. Conclusion Fuzheng Guben Recipe I can significantly improve the immune function in patients of advanced non small cell lung cancer with syndrome of lung and spleen deficiency,alleviate the toxic and
