银屑平丸对银屑病急性期患者表皮组织VEGF、KGF和AREG的影响

摘　　要：目的观察银屑平丸对银屑病急性期患者表皮组织双调蛋白(AREG)、角质形成细胞生长因子(KGF)及血管内皮生长因子(VEGF)的影响。方法将316例银屑病急性期患者按入院先后分为对照组和银屑平丸组。对照组口服阿维A、窄谱中波紫外线局部(或全身照射)及外用卤米松软膏,银屑平丸组在此治疗基础上加服银屑平丸。采用皮损面积和严重程度指数(PASI)对病变皮肤评分;ELISA法定量测定治疗前后表皮组织中AREG、KGF和VEGF蛋白量;测定表皮微血管密度(MVD),流式细胞仪检测静脉血清体外培养人永生角质形成细胞株(HaCaT)细胞增殖和细胞周期,并观察临床疗效。结果治疗4周后,银屑平丸组AREG、KGF、VEGF和PASI均显著降低,HaCaT细胞抑制率提高、S期细胞数降低,有效率为83.54%,与对照组比较,差异有统计学意义(P<0.05)。结论银屑平丸通过降低表皮组织AREG、VEGF含量,抑制KGF诱导的HaCaT细胞增殖来减少表皮细胞角化和皮下组织血管新生,降低微血管的异常增生而达到治疗银屑病急性期的目的。Objective： To observe the drugamphiregulin Yinxieping Pill on epidermal tissue of patients with a-cute psoriasis （AREG）, keratinocyte growth factor （KGF） and vascular endothelial growth factor （VEGF） effect. Methods： 316 cases of patients with acute psoriasis were divided into control group and Yinxieping pill group. The control group oral acitretin, A NB-UVB （local or systemic irradiation） and topical ointment Halometasone therapy, Ping pill group and psoriasis in Yinxieping pill. The psoriasis area and severity index （PASI） score of skin lesion;ELISA quantitative determination of AREG, KGF and VEGF protein before and after treatment in the epidermis of skin;Determination of microvessel density （MVD）, flow cytometry detection serum in vitro cultured human immortalized keratinocytes strain （HaCaT） cell proliferation and cell cycle, and to observe the clinical curative effect. Results： After treatment, Yinxieping pill group AREG, KGF, VEGF and PASI were significantly decreased, HaCaT cell inhibition rate increased, the number of S cells decreased and the effective rate was 83.54%, compared with the control group and the same group before treatment（P〈0.05）, the difference was sta-tistically significant. Conclusion： Yinxieping pill by decreasing AREG and VEGF content of epidermal tissue, inhibit KGF induced proliferation of HaCaT cells to reduce angiogenesis of keratinized cells and subcutaneous tis-sue, hyperplasia of epidermal lesions, reduce microvascular and to treat psoriasis like pathological changes in a-cute stage.
