摘  要:为了分析骨关节结核患者TNF-α、TGF-β炎症因子的表达及其临床应用价值,本研究以我院2015年1月~2016年9月期间收治的50例骨关节结核患者为研究对象,按骨关节结核病变分为增生性病变组和干酪性病变组,分析两组患者血清TNF-α、TGF-β炎症因子阳性表达差异;以30例健康人群为对照组,分析TNF-α、TGF-β对骨质骨量的影响。结果表明,TNF-α、TGF-β在不同病变骨关节结核组织中呈现高低不同的阳性表达,TNF-α、TGF-β在增生性病变结核组织中的表达水平显著高于干酪性病变结核组织中的表达水平(p<0.05),TNF-α、TGF-β在增生性病变的坏死性结核组织中和非坏死性结核组织中的表达水平无明显差异性(p>0.05),但TNF-α、TGF-β在干酪性病变的坏死性结核组织中的表达水平显著高于干酪性病变的非坏死性结核组织中的表达水平(p<0.05)。增生性病变患者和干酪性病变患者的骨关节结核组织中的成骨细胞数均明显低于正常人(p<0.05),而破骨细胞数则明显高于正常人(p<0.05),增生性病变患者骨关节结核组织中的成骨细胞数明显低于干酪性病变患者(p<0.05),而破骨细胞数则明显高于干酪性病变患者(p<0.05)。由此可知,骨关节结核患者TNF-α、TGF-β炎症因子在不同病变骨关节结核组织中存在不同程度的表达,对患者病情发展及患者骨质骨量均有着重要的影响。In order to analyze the expression and study the clinical application value of inflammatory factors TNF-α and TGF-βin patients with joint tuberculosis, we selected 50 cases of patients with joint tuberculosis that admitted in our hospital from January 2015 to September 2016 as the research object, who were divided into the proliferative lesion group and caseous lesion group according to the tuberculosis of bone and joint, and then we analyzed the difference in positive expression of serum TNF-α and TGF-β inflammatory factors of the two groups. Also, other 30 cases of healthy people were selected to form the control group to analyze the effect of TNF-α and TGF-β on bone. The results indicated that TNF-α and TGF-β showed different levels of positive expression in different lesions of bone and joint tuberculosis tissue. The expression level of TNF-α and TGF-β in the proliferative lesion group was significantly higher than that in the caseous lesion group (p 〈0.05), but they had no obvious difference in necrotic tuberculosis tissue and non-necrotic tuberculosis tissue of proliferative lesion (p〉 0.05), while the expression level of TNF-αand TGF-β in the necrotic tuberculosis tissue of caseous lesion groupwas significantly higher than that in the non-necrotic tuberculosis tissue of caseous lesion (p〈0.05). The number of osteoblasts in bone and joint tuberculosis tissue of patients with proliferative lesions and caseous lesions was significantly lower than normal people (p〈0.05), while the number of osteoclasts was obviously higher than that of normal people (p 〈0.05). In addition, the number of osteoblasts in bone and joint tuberculosis tissue of proliferative lesion group was lower than caseous lesion group (p〈0.05), but the number ofosteoclasts was higher than caseous lesion group (p 〈0.05).
