摘 要:目的比较脑干听觉诱发电位(BAEP)及NBNA评分法在极低出生体质量儿(VLBWI)的神经发育评估中的价值。方法回顾性分析2015年1月—2016年7月新生儿重症监护室收治的符合条件的VLBWI 78例,并于纠正胎龄42 w时行BAEP及NBNA评分,并对数据作统计学处理。结果 78例符合条件的VLBWI中,在纠正胎龄42 w时行NBNA不通过者25例,行BAEP显示不通过者28例,随访至纠正6月龄时有13例MDI<70,有10例PDI<70,即合计有神经发育损害(NDI)23例。经统计学分析,BAEP、NBNA评分监测结果均与NDI监测结果无统计学差异,提示VLBWI脑干听觉诱发电位及NBNA评分在评估NDI发生率上无统计学差异。结论在早期可以行BAEP或NBNA的方法预测患儿神经发育情况,早期干预,以减少NDI的发生。Objective To compare the value of brainstem auditory evoked potential (BAEP) and NABA scoring in the neural development evaluation of very low birth weight infants (VLBWI). Methods Seventy - eight VLBWI, who were admitted to neonatal intensive care unit in our hospital from January 2015 to July 2016, were analyzed retrospectively. The BAEP and NBNA scores were evaluated at the 42nd week of corrected gestation, and the data were statistically analyzed. Re- suits Among the 78 VLBWI, twenty - five failed in NBNA at the 42nd week of corrected gestation, twenty - eight failed in BAEP. Follow - up was conducted until 6 months of corrected age, and 13 cases had MDI below 70, ten had PDI below 70, and namely 23 had total neurological damage(NDI). According to the statistical analysis, there was no significant difference between BAEP, NBNA scores and NDI monitoring results, suggesting that there was no significant difference in the incidence of NDI between brainstem auditory evoked potentials and NBNA scores of VLBWI. Conclusion BAEP or NBNA can be used to predict the development of children's nervous system in early periods and early intervention is helpful to reduce the inci- dence of NDI.
