盲摇刻度标尺在预防呼吸机相关性肺炎中的应用

摘　　要：目的探讨盲摇刻度标尺在预防呼吸机相关性肺炎(VAP)中的效果。方法收集2015年9月—2016年6月在我院就诊的414例ICU气管插管患者,随机分为2组。对照组采用普通多功能病床,粗略地调节机械通气患者的床头抬高角度;实验组则将盲摇刻度标尺运用于普通多功能病床中,直观精确地把握床头抬高角度。观察2组患者的VAP发生率及应用呼吸机和住ICU天数。结果实验组3个季度的VAP发生率分别为14.4%、11.0%、3.6%,对照组的3个季度VAP发生率分别为27.0%,19.8%,13.0%,两组比较差异有统计学意义(P<0.05)。实验组应用呼吸机和住ICU天数少于对照组,两组比较差异有统计学意义(P<0.05)。结论运用盲摇刻度标尺能保证精确地为患者采取抬高床头30°~45°体位,促进分泌物从气管经口排出或吸出,有利于咳嗽和深呼吸,从而有效地降低了VAP发生。Objective Discussion on the effect of blind pitch scale in VAP prevention. Methods 414 patients with ICU who were admitted to our hospital from September 2015 to June 2016 were collected and randomly divided into 2 groups. The control group was treated with general multifunctional bed, which was used to adjust the bed elevation angle; The experi- mental group was used to measure the height of the bed by using the scale of the blind swing in the general muhifunctional sickbed. The incidence of VAP in two groups and the number of days of using ventilator and ICU were observed. Results The incidence of VAP in the three groups was 14.4%, 11.0% and 3.6% respectively. The incidence of VAP in the three groups was 27.0%, 19.8% and 13.0% respectively. The difference was statistically significant （P 〈0. 05）. The number of days of ventilator and ICU was less than that of the control group, and the difference was statistically significant （P 〈 0.05 ）. Conclusion The use of blind swing scale can ensure accurate for patients to take up the bed 30 degrees to 45 degrees posi- tion, promote secretion from the trachea through the mouth or suck out, is conducive to cough and deep breathing, thus effec- tively reducing the occurrence of VAP.
