自拟活血通脉汤联合外科手术对单纯性下肢浅静脉曲张患者氧化-抗氧化系统及血液流变学的影响

摘　　要：目的:观察自拟活血通脉汤联合外科手术对单纯性下肢浅静脉曲张患者氧化-抗氧化系统及血液流变学的影响。方法:将106例单纯性下肢浅静脉曲张患者随机分为观察组(n=53例)和对照组(n=53例),对照组给予外科手术治疗,观察组在对照组治疗基础上术后给予自拟活血通脉汤治疗,疗程为4周;观察两组治术后皮下血肿、硬结、瘀斑及患肢麻木疼痛等并发症的发生情况;观察两组治疗前后静脉疾病临床严重程度评分(RVCSS评分)、静脉功能不全生活质量问卷中的VEINES-Sym评分及VEINES-QOL评分、洪堡静脉曲张严重程度评分(HVVSS评分)变化情况;检测两组治疗前后氧化-抗氧化系统及血液流变学相关指标。结果:观察组治疗后2周、4周的皮下血肿、硬结、瘀斑及患肢麻木疼痛发生率低于对照组(均P<0.05);两组治疗后2周、4周的RVCSS评分、VEINES-Sym评分、VEINES-QOL评分、HVVSS评分均有改善,观察组上述评分的改善情况优于对照组(均P<0.05);观察组治疗后2周、4周的临床疗效优于对照组(均P<0.05);两组治疗后过氧化氢酶(CAT)、超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-PX)升高,丙二醛(MDA)水平降低,观察组上述指标改善情况优于对照组(均P<0.05);观察组治疗后全血黏度(低、中、高切)、血浆黏度及红细胞比积均有低于对照组(均P<0.05)。结论:外科手术治疗单纯性下肢浅静脉曲张的基础上,采用自拟活血通脉汤治疗能够降低术后并发症,并改善患者症状,提高疗效及生活质量,抑制氧化应激反应并改善血液流变学,从而促进患者康复。Objective ： To observe the influence of self - made Huoxue Tongmai Decoction and surgery on oxidation - antioxi- dant system and blood rheology of patients with lower extremity varicose veins. Methods：A total of 106 patients with single lower extremity varicose veins were randomly divided into observation group（ n = 53 cases）and control group（ n = 53 cases）and observa- tion group. The control group used surgical treatment. The observation group were treated on the basis of ccontrol group with self -made Huoxue Tongmai Decoction, treatment of 4 weeks. Observe two groups＇ postoperative subcutaneous hematoma, bruises, indu- ration and the occurrence of complications such as limb numbness pain. Observe the two groups＇ venous disease severity score （RVCSS score） ,venous insufficiency of the quality of life questionnaire VEINES Sym score and VEINES- QOL score, Humboldt varices severity score （HVVSS score）changes before and after the treatment. Detect the two groups＇ oxidation treatment -antioxi- dant system and the related parameters of blood rheology. Results ：2 and 4 weeks after treatment, the observation group＇s subcuta- neous hematoma, bruises,induration and limb numbness pain incidence rate were lower than the control group＇s （P 〈 0. 05）. Two groups＇ RVCSS score VEINES Sym score,VEINES QOL score and HVVSS score at 2 and 4 weeks after treatment had improved. The improvement of the observation group＇s score was better than the control group＇s（P 〈 0. 05 ）. Observation group＇s clinical curative effect was better than that of control group 2 and 4 weeks after treatment （ P 〈 0. 05 ）. The catalase （ CAT）, superoxide dis- mutase（ SOD）, glutathione peroxidase（ gsh- px）and malondialdehyde（MDA） levels after treatment observation group were better than those of control group（ P 〈 0. 05 ）. tier treatment, observation group＇s whole blood viscosity （low, medium and high shear）, plasma viscosity and erythrocyte were lower than the control group
