摘 要:目的:基于清热解毒化浊片对肠源性内毒素血症(Intestinal endotoxemia,IETM)大鼠胰损伤作用探讨中医整体观对临床治疗的指导意义。方法:将70只SD大鼠随机分为正常组10只、模型组60只。模型组采用乙醇及高脂饲料喂养建立大鼠IETM胰腺损伤模型,8周末随机取20只模型大鼠确定造模成功,剩余40只大鼠分为模型组和清化组各20只,连续给药4周后,分别从腹主动脉采血检测内毒素,取胰腺组织予免疫组化法观察各组胰腺血小板内皮细胞黏附分子-1(PECAM-1)、白细胞介素-1(IL-1)及白细胞介素-4(IL-4)的表达,HE染色观察胰腺病理形态改变。结果:与模型组比较,清化组血浆内毒素、PECAM-1、IL-1下降(P<0.01),IL-4上升(P<0.01或P<0.05);与正常组比较,模型组血浆内毒素、PECAM-1及IL-1上升(P<0.01);模型组胰腺腺泡间距增宽且结构破坏,有炎性细胞浸润等改变;正常组及清化组改变不明显。结论:清热解毒化浊片能降低内毒素水平,减轻全身炎性反应,下调PECAM-1的表达,改善胰腺微循环及组织损伤,而清热解毒化浊片疗效的物质基础是中医整体观在治疗中的体现。Objective :To investigate the guiding significance of traditional Chinese medicine (TCM)holistic view in clinical treatment based on the effect of Qingre Jiedu Huazhuo tablets on pancreatic injury in rats with intestinal endotoxemia (IETM). Methods : A total of 70 Sprague - Dawley rats were randomly divided into normal group with 10 rats and model group with 60 rats. A rat model of pancreatic injury in IETM was established by alcohol and high - fat feed. At the end of week 8,20 rats were randomly selected to confirm whether the model was established successfully, and the other 40 rats were divided into model group and Qingre Jiedu Huazhuo group, with 20 rats in each group. After administration for 4 consecutive weeks, blood samples were taken from the abdominal aorta to measure endotoxin;pancreatic tissues were collected and immunohistochemistry was used to measure the expression of platetet/endothelial cell adhesion molecule - 1 ( PECAM - 1 ), interleukin - 1 ( IL - 1 ), and interleukin - 4 ( IL - 4) in the pancreas ; HE staining was performed to observe the pathomorphological changes of the pancreas. Results : Compared with the model group, the Qingre Jiedu Huazhuo group had significant reductions in plasma endotoxin, PECAM - 1, and IL - 1 ( P 〈 0.01 ) and a significant increase in IL - 4 ( P 〈 0. 01 orP 〈 0.05 ). Compared with the normal group,the model group had significant increases in plasma endotoxin,PECAM -1, and IL- 1 (P 〈0.01 ). The model group showed widened space between pancreatic acinars, structural damage, and changes including in- flammatory cell infiltration,while there were no significant changes in the normal group and the Qingre Jiedu Hua- zhuo group. Conclusion:Qingre Jiedu Huazhuo tablets can reduce the level of endotoxin, alleviate systemic inflammatory response, downregulate the expression of PECAM -1 jury.
