摘  要:目的探讨C-P水平与新诊断T2DM周围神经病变(DPN)的关系。方法根据是否合并DPN,将677例新诊断T2DM患者分为DPN组264例,不合并DPN(NDPN)组413例。收集两组临床基本资料及生化指标,DPN的影响因素行Spearman相关分析及Logistic二元回归分析。结果DPN组年龄、HbA_1c高于NDPN组,TG、FC-P、1 hC-P、2 hC-P水平均低于NPDN组(P<0.05)。DPN与年龄、HbA_1c呈正相关,与TG、FC-P、1 hC-P、2 hC-P呈负相关(P<0.05)。Logistic二元回归分析显示,年龄和FC-P是DPN发生的独立危险因素。结论新诊断T2DM患者C-P水平的下降与DPN的发生密切相关,其中FC-P为独立危险因素,C-P对于新诊断DPN的诊治有重要意义。Objective To explore the relationship between C-peptide level and peripheral neuropathy in patients with newly diagnosed type 2 diabetes mellitus. Methods Total of 677 patients with newly diagnosed type 2 diabetes mellitus (T2DM) were enrolled in this study and divided into two groups according to whether complicated with peripheral neuropathy: peripheral neuropathy group (DPN,n=264) and non peripheral neuropathy group (NDPN,n=413). The basic clinical data and biochemical indexes of all the subjects were collected and compared between the two groups. Spearman correlation analysis and logistic regression analysis were used to evaluate the influencing factors for DPN. Results The age and HbA1 c were higher,while TG, FC-P, 1 hC-P, and 2 hC-P levels were lowerin DPN group than in NDPN group. Logistic regression analysis showed that age and FC-P were independent risk factors for DPN. Conclusion The reduction of C-P level in patients with newly diagnosed T2DM is closely related to the development of DPN. And FC-P is an independent risk factor for DPN. C-P is important in the DPN therapy in patients with newly diagnosed T2DM.
