摘 要:目的观察健康人群中血清性激素结合球蛋白(SHBG)水平和胰岛β细胞分泌功能的关系,探讨可能的机制。方法选取门诊体检糖耐量正常(NGT)者30名,检测血清SHBG水平、HbA_1c、血脂谱、肝肾功能等生化指标。行OGTT+胰岛素释放试验,评估胰岛β细胞功能(HOMA-β)、早期相胰岛素分泌功能和晚期相胰岛素分泌功能。结果平均SHBG为(115.4l±59.37)nmol/L,校正性别、年龄后,NGT者SHBG与胰岛β细胞早期相分泌功能(r=-0.40,P=0.028)、晚期相分泌功能(r=-0.39,P=0.033)呈负相关;与HOMA-β无相关性(r=-0.28,P=0.131)。校正HOMA-IR后,早期相胰岛素分泌功能和晚期相胰岛素分泌功能与SHBG无相关性(r=-0.26,P=0.172;r=-0.24,P=0.214)。以SHBG为因变量,以HOMA-β、早期相胰岛素分泌功能、晚期相胰岛素分泌功能和HOMA-IR为自变量,行多元回归分析,结果显示,各项指标均非SHBG的影响因素(P>0.05)。结论在NGT人群中,SHBG与胰岛β细胞分泌功能相关,但这一相关性可能是继发于IR。Objective Low sex hormone binding globulin level is one of the important causes of type 2 diabetes mellitus. But the precise mechanisms that sex hormonebinding globulin leading to type 2 diabetes is not clear. This article is to observe the relationship between serum sex hormone binding globulin levels in healthy population and islet beta cell secretion function. Methods 30 healthy subjects were enrolled in the study. We detected the biochemical index of serum sex hormone binding globulin levels, glycosylated hemoglobin, lipid profile and liver function. All subjects underwent an oral glucose tolerance test and insulin releasing test to evaluate the pancreatic islet beta cell function index, early phase insulin secretion and late phase insulin secretion. Results The average levels of sex hormone binding globulin in healthy population was (115.41 ± 59.37) nmol/L in our study. Adjusted for gender, age, SHBG was significantly negatively correlated with early phase beta cell secretion (r=-0. 40, P=0. 028), late phase insulin secretion (r=-0.39, P=0. 033) in healthy subjects and are negative correlation trend with HOMA-β, but did not reach statistical significance (r= 0. 28, P=0. 131). After correction for HOMA-IR, early phase insulin secretion and late phase insulin secretion function vanishing correlation with SHBG (r=-0. 26, P=0. 172 and r=-0. 24, P=0. 214). Conclusions In healthy people with normal glucose tolerance, SHBG is correlated with islet beta cell secretion function, but this correlation is likely secondary to insulin resistance.
