两种内固定治疗股骨远端关节内骨折的疗效比较

摘　　要：目的探讨微创内固定系统(LISS)钢板内固定和解剖钢板内固定治疗股骨远端关节内骨折患者的效果。方法将83例股骨远端关节内骨折患者随机分为LISS组(42例)和解剖钢板组(41例)。比较两组患者的手术相关指标和术后关节功能恢复指标。结果术中出血量、手术切口长度、住院时间、骨折愈合时间LISS组均少于解剖钢板组,差异有统计学意义(P<0.05);疼痛、行走能力、关节稳定性、膝关节活动度、主观评分总分LISS组均高于解剖钢板组,差异均有统计学意义(P<0.05)。髁部变宽、关节面塌陷、成角畸形、放射学总分两组比较差异均无统计学意义(P>0.05)。采用HSS系统评价膝关节功能优良率:LISS组为88.1%,高于解剖组的73.2%,但差异无统计学意义(P>0.05)。结论采用LISS钢板内固定治疗股骨远端关节内骨折手术创伤更小,术后恢复更快,对患者的膝关节功能恢复更加有利。Objective To evaluate the effect of anatomical plate internal fixation and less invasive stabilization system （LISS） plate internal fixation in the treatment of distal femoral intra-articular fractures.Methods The 83 patients with distal femoral intra-articular fractures were randomly divided into LISS group （n=42） and anatomical plate group （n=41）.The two groups of patients with surgical related indicators,postoperative joint function recovery index were compared.Results The intraoperative blood loss,incision length,length of stay and fracture healing time in LISS group were lower than in anatomical plate group （P〈0.05）.Pain,walking ability,joint stability,knee activity and subjective score of LISS group were higher than those of anatomical plate group,the difference was statistically significant （P〈0.05）.The condylar widening,articular surface collapse,angular deformity,radiology total scares had no significant difference between the two groups （P〉0.05）.According to HSS system evalution,the excellent-good rate of knee function was 88.1% in LISS group,which was higher than that in anatomical group （73.2%）,but the difference was not statistically significant （P〉0.05）.Conclusions The use of LISS plate internal fixation in the treatment of distal femoral intra-articular fractures patients with distal surgery less wound,faster recovery after surgery,the patient＆#39;s knee recovery is more favorable.
