重建鼻翼外脚长度及高度矫正唇裂术后鼻畸形

摘　　要：目的探索一种以延长鼻翼外脚长度、高度为主,辅以矫正错位的组织,来重建并矫正唇裂术后鼻翼外脚的畸形,以达到鼻畸形的矫正的手术方法。方法松解鼻翼外脚基底及梨状孔外缘,动员鼻面沟处组织,通过贯穿固定,来延长鼻翼外脚长度、高度;将口轮匝肌上部及提上唇肌填充鼻翼外脚基底使其抬起;将重建后的鼻翼外脚向内上方旋转矫正鼻翼外脚向外下移位。结果我院以该方法进行了45例手术,近期效果满意,随访三个月到四年满意。结论鼻翼外脚的长度及高度的重建,对唇裂术后鼻翼畸形矫正是至关重要的。Objective To investigate the method of extending the length and height of the outside corner of the nose with correcting dislocation organization for the treatment on nasal deformity caused by surgery of unilateral cleft lip. Methods Release the base of the outside corner of the nose and the outside part of the piriform aperture;Extend the length and height of the outside corner of the nose using tissue of the border of nose and face;Lift up the base of the outside corner of the nose using orbicularis oris muscle and the upper lip nose muscle;Above inward rotation the the outside corner of the nose to correct the shift. Results 45 cases were followed up for 3 months to 4 years. Satisfactory results were achieved in all of cases. Conclusion The reconstruction of the length and height of the outside corner of the nose is important in nasal deformity caused by surgery of unilateral cleft lip.
