自制肩部护带在小切口大汗腺剪除术中的应用

摘　　要：目的探讨自制的肩部护带在大汗腺小切口剪除术后包扎中的应用。方法回顾性分析我科138例大汗腺患者采用小切口剪除术治疗,术后包扎时不用传统的绷带包扎,采用自制的带有弹力的肩部护带包扎,并将其和之前传统的绷带包扎的150例患者对比。结果采用自制肩部护带包扎方法的患者均未出现末梢肿胀淤青等循环障碍,1例皮下血肿局部皮肤坏死,5例患者因肩关节早期活动出现皮肤表面水泡经换药后好转,无明显疤痕,15人NRS疼痛评分超过4分影响睡眠,而传统绷带包扎的患者,有9例出现皮下血肿局部皮肤坏死,26例患者出现局部皮肤淤青发生张力性水泡,46人NRS疼痛评分超过4分。通过SPSS19.0分析,两者在血肿发生率,局部张力性水泡发生率及疼痛评分上有显著差异,有统计学意义。结论自制的肩部护带包扎使患者术后痛苦减轻,安全可靠,腋窝皮肤外型美观自然,肩关节活动无影响,值得推广。Objective To explore the application of self made shoulder protecting bandage in the small incision radical operation of big sweat gland. Methods Retrospective analysis of 138 cases of sweat glands in our department who were treated with small incision radical operation,After the operation, self-made shoulder protecting bandage with elastic was used to replace the traditional bandage for dressing therapy ,and compared with 150 cases were treated with of traditional bandaging .Results Patients with a self-made shoulder protecting bandage, peripheral swelling, bruising, circulatory disorder were not seen in all patients. Local skin necrosis in 1 cases because subcutaneous hematoma ,5 patients for early activity of shoulder joint appear skin blisters on the surface and cured after dressing change, no obvious scar,15 cases NRS pain score more than 4 points and afect sleep, The patients were treated with of traditional bandaging, there were 9 cases with local skin necrosis because subcutaneous hematoma , 26 cases ofpatients with local skin bruising blisters occurred, 46 cases NRS pain score more than 4 points. By SPSS19.0 , there were signifcant diferences in the incidence of hematoma, local skin bruising blisters and pain scores, there was statistical signifcance between the two groups. Conclusion We can reduce the pain of patients by using the new bandage method,and postoperative axillary skin shows natural appearance, shoulder joint activities freely. it is worth promoting.
