两种分期手术模式对治疗小儿重度尿道下裂的临床应用研究

摘　　要：目的探讨分期管形包皮岛状皮瓣术式与分期尿道板重建卷管术式两种手术模式对小儿重度尿道下裂的治疗效果。方法选取2012年1月—2015年12月我院收治的重度尿道下裂患儿80例,随机分成观察组和对照组,各40例。观察组患儿行分期管形包皮岛状皮瓣术式治疗,对照组行分期尿道板重建卷管术式治疗,观察两组手术成功率和术后并发症。结果观察组治疗成功率92.50%(37例),对照组治疗成功率72.50%(29例),观察组明显高于对照组,比较差异有统计学意义(P<0.05);观察组术后出现尿瘘3例,尿道狭窄1例,术后并发症发生率10.00%;对照组术后出现尿瘘6例、尿道狭窄4例、尿道憩室3例,发生率32.50%。观察组术后并发症发生率明显低于对照组,比较差异有统计学意义(P<0.05)。结论分期管形包皮岛状皮瓣术式治疗重度尿道下裂,术后成功率高,不良反应少,手术效果美观,具有临床推广价值。Objective To study the stage tubular preputial Island fap for urethral plate and stage-two operation modes on volume of reconstruction treatment of severe hypospadias in children.Methods From January 2012-admitted to hospital in December 2015 80 cases of children with severe hypospadias, randomized study group and control group, each of 40 cases.Observer groups staging tubular preputial Island flap for the treatment, control group stage volumes of urethral plate reconstruction surgery, were observed in all surgical success rates and complications.Results Group treatment success 92.5% （37 cases）, treatment success rates in the control group 72.5% （29 cases）, the Observer Group was signifcantly higher than the control group, a statistically signifcant diference （P 〈 0.05）;Observation of postoperative urinary fstula in 3 cases, 1 cases of urethral stenosis, postoperative complication rate of 10%;Control urinary fstula after operation of 6 cases, 4 cases of urethral stricture, urethral diverticulum in 3 cases, the incidence of 32.5%.Observation of postoperative complication rate was significantly lower than the control group, a statistically significant diference （P 〈 0.05）.Conclusion Staging tubular preputial Island fap for the treatment of severe hypospadias, postoperative high success rate, fewer side efects, efect of appearance, has clinical value.
