特发性肺纤维化患者血清肿瘤标志物水平与疾病严重程度关系

摘　　要：目的:分析特发性肺纤维化(Idiopathic pulmonary fibrosis,IPF)患者血清肿瘤标志物水平与疾病严重程度的关系。方法:选取2013年3月—2016年3月74例IPF患者,检测患者血清甲胎蛋白(AFP)、癌胚抗原(CEA)、神经元特异性烯醇化酶(NSE)、细胞角蛋白19片段(Cyfra21-1、)、鳞状上皮细胞内抗原(SCC)、糖类抗原19-9(CA19-9)、CA15-3、CA125等肿瘤标志物,分析上述指标与患者肺功能、全肺纤维化高分辨率CT(high-resolution CT,HRCT)评分的相关性。结果:IPF患者FEV1为57.58±5.46、FVC为55.19±5.82、FEV1/FVC为85.04±2.28,患者HRCT评分为(2.26±0.43)分,Pearson相关性分析示,CEA、CA19-9、CA15-3与患者肺功能呈负相关,CEA、CA19-9、CA153、CA125与患者全肺纤维化HRCT评分呈正相关(P<0.05)。结论:IPF患者血清CEA、CA19-9、CA153、CA125水平与其疾病严重程度具有一定关联,据此了解患者肺间质受损程度有望为临床诊治及病情控制提供参考。Objective This study objective was to investigate the correlation between serum tumor markers and severity of idiopathic pulmonary fbrosis （IPF） in patients with idiopathic pulmonary fbrosis （IPF）. Methods： Serum alpha-fetoprotein （AFP）, carcinoembryonic antigen （CEA）, neuron-specifc enolase （NSE）, cytokeratin 19 （Cyfra21-1）, squamous cell carcinoma （SCC）, carbohydrate antigen 19-9 （CA19-9）, CA15-3, CA125 and other tumor markers were detected in 74 patients with IPF treated from March 2013 to March 2016, the correlation between the high-resolution CT （HRCT） scores of patients’ pulmonary function and pulmonary fbrosis and above indicators were analyzed. Results： FEV, FVC and FEV1/FVC of IPF patients were 57.58±5.46, 55.19±5.82 and 85.04±2.28, respectively. HRCT score of IPF patients was （2.26±0.43）. Pearson correlation analysis showed that there was a negative correlation between CEA, CA19-9, CA15-3 and pulmonary function （P〈0.05）. There was a positive correlation between CEA, CA19-9, CA153 and CA125 and HRCT scores of pulmonary fbrosis in patients （P 〈0.05）. Conclusions： The level of serum CEA, CA19-9, CA153 and CA125 in IPF patients is correlated with the severity of IPF. It is understood that the degree of impaired interstitial lung disease is expected to provide reference for clinical diagnosis and treatment.
