探析结肠癌合并2型糖尿病患者IGF-2及IGFBP-2升高与结肠癌复发的关系

摘　　要：目的探讨结肠癌合并2型糖尿病患者胰岛素样生长因子2(IGF-2)及胰岛素样生长因子结合蛋白2(IGFBP-2)升高与结肠癌复发的关系。方法 90例结肠癌合并2型糖尿病患者分别于术后随访1年检测血清IGF-2、IGFBP-2水平,并根据患者术后1年内复发情况分为复发组和未复发组,比较两组患者的血清IGF-2、IGFBP-2水平,并计算血清IGF-2、IGFBP-2水平与结肠癌复发的相关性,描绘血清IGF-2、IGFBP-2与结肠癌复发的受试者工作特征(ROC)曲线,计算血清IGF-2、IGFBP-2对结肠癌复发的预测灵敏度、特异度。结果 90例结肠癌合并2型糖尿病患者中共有24例患者出现结肠癌复发,其复发率为26.67%,复发组患者的血清IGF-2、IGFBP-2水平均明显高于未复发组患者(P<0.05);血清IGF-2、IGFBP-2水平与结肠癌复发均呈正相关(P<0.05),;分析ROC曲线发现,血清IGF-2以223.0 ng/ml作为临界值,曲线下面积(AUC)为0.653,灵敏度、特异度分别为84.8%、50.9%,血清IGFBP-2以221.0 ng/ml作为临界值,AUC为0.633,灵敏度、特异度分别为78.8%、56.1%。结论结肠癌合并2型糖尿病患者血清IGF-2、IGFBP-2的升高与结肠癌复发密切相关,可对结肠癌复发进行预测。Objective To explore the correlation between rising insulin-like growth factor 2（IGF-2） and insulin-like growth factor binding protein 2（IGFBP-2） and colon cancer recurrence in colon cancer complicated with type 2 diabetic mellitus patients. Methods A total of 90 colon cancer complicated with type 2 diabetic mellitus patients had serum IGF-2 and IGFBP-2 level detected in postoperative 1 year follow-up, and they were divided by recurrence situation in postoperative 1 year follow-up into recurrence group and non recurrence group. Comparison were made on serum IGF-2 and IGFBP-2 level in two groups, and calculation was made on correlation between serum IGF-2 and IGFBP-2 level and colon cancer recurrence by painting receiver operating characteristic（ROC） curves of serum IGF-2 and IGFBP-2 level and colon cancer recurrence. Calculation was made on sensitivity and specificity of serum IGF-2 and IGFBP-2 for prediction of colon cancer recurrence. Results Among 90 colon cancer complicated with type 2 diabetic mellitus patients, there were 24 cases with colon cancer recurrence, with recurrence rate as 26.67%. Recurrence group had obviously higher serum IGF-2 and IGFBP-2 level than non recurrence group（P〈0.05）. Serum IGF-2 and IGFBP-2 level were positively correlated to colon cancer recurrence（P〈0.05）. ROC curve analysis showed that serum IGF-2 had 223.0 ng/ml as critical value, with area under the curve（AUC） as 0.653, sensitivity and specificity respectively as 84.8% and 50.9%, while serum IGFBP-2 had 221.0 ng/ml as critical value, with AUC as 0.633, sensitivity and specificity respectively as 78.8% and 56.1%. Conclusion Rising IGF-2 and IGFBP-2 are closely correlated to colon cancer recurrence in colon cancer complicated with type 2 diabetic mellitus patients, and they can predict recurrence of colon cancer.
