基于步积式采血法的择期手术患者贮存式自身输血的疗效评价

摘　　要：目的评估基于步积式采血法的贮存式自身输血(preoperative autologous blood donation,PABD)对择期手术患者的临床疗效。方法选取2015年1月至2016年6月在西南医科大学附属医院行择期手术的患者200例,均符合实施PABD的标准。实验组100例,实施PABD,术前采用步积式采血法采集自身血液进行储备,术中回输;对照组100例,患者在术中实施异体输血。观察和统计两组患者的住院时间、输血不良反应、术后感染发生率及预后情况。结果与对照组相比,实验组的住院时间、输血不良反应发生率均明显降低(P<0.05),预后显著改善(P<0.05);两组术后感染发生率比较,差异无统计学意义(P=0.205)。结论对于择期手术患者而言,基于步积式采血法的PABD的临床疗效优于异体输血。Objective To evaluate the clinical efficacy of preoperative autologous blood donation （PABD）in patients of selective operations by means of step-by-step blood sampling.Methods 200 selective operation patients, who were treated from January of 2015 to June of 2016 in The Affiliated Hospital of Southwest Medical University and compliant with the implementation standard of PABD,were selected as research objects and divided into the experiment group and the control group,and each group consisted of 100 patients.The patients of the experiment group were transfused intraoperatively with their autologous blood collected before operation by means of step-by-step blood sampling, while the patients of the control group were given allogeneic blood transfusion during operation.Then the two groups were compared in terms of the average length of hospital stay,adverse transfusion reactions,postoperative infection rate and prognosis.Results There were shorter hospital stays,lower rate of adverse transfusion reactions and better prognosis in the experiment group than in the control group,and the differences were statistically significant （P〈0.05 ）.There were no significant differences in the postoperative infection rate between the two groups （P=0.205）.Conclusion The clinical efficacy of PABD by means of step-by-step blood sampling is better than that of allogeneic blood transfusion in selective operation patients.
