益气活血汤治疗早期糖尿病肾病临床观察

摘　　要：目的观察益气活血汤治疗早期糖尿病肾病(DN)的临床疗效。方法将80例早期DN患者随机分为2组,治疗组45例予益气活血汤治疗,对照组35例予缬沙坦胶囊治疗。2组均治疗16周。观察2组治疗前后血糖[空腹血糖(FPG)、餐后2h血糖(2hPG)、糖化血红蛋白(HbA_1c)]、血脂[血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)]、肾功能[肌酐(Cr)、尿素氮(BUN)]及D-二聚体,细胞炎症因子肿瘤坏死因子α(TNF-α)、白细胞介素(IL)-6、IL-18变化。结果2组治疗后FPG、2hPG、HbA_1c、TC、TG、LDL-C、Cr、BUN、D-二聚体、TNF-α、IL-6、IL-18均较本组治疗前降低(P<0.05),HDL-C升高(P<0.05)。结论益气活血汤治疗早期DN,具有改善微循环、抑制细胞炎性因子、预防微血栓形成的作用,可降低早期DN患者体内D-二聚体水平。Objective To observe the clinical effect of tonifying, qi and activating - blood decoction on the treatment of early diabetic nephropathy （DN）. Methods 80 patients with early DN were randomly divided into two groups. The treatment group （ n = 45 ） was treated by tonifying - qi and activating - blood decoction. The control group （n = 35） was treated by valsartan capsule. The treatment course lasted for 16 weeks. The changes of the blood- glucose including fasting plasma glucose （FPG）, 2 h plasma glucose （2 hPG） and glycosylated hemoglobin （HbA1 c）, the blood lipid including total Cholesterol （TC）, triglycerides （TG）, low- density lipoprotein cholesterol （LDL- C）, high- density lipopratein cholesterol （HDL -C）, the renal function including serum creatinine （Cr）, blood urea nitrogen （ BUN）, d - dimer （ D - D）, tumor necrosis factor alpha （ TNF - α）, interleukin - 6 （ IL - 6 ） and interleukin- 18 （IL- 18）before and after treatment were observed in two group. Results The FPG, 2 hPG, HbAlc, TC, TG, LDL- C, Cr, BUN, D- D, TNF -α, IL-6 and IL- 18 after treatment were decreased in two groups （ P 〈 0. 05 ）, and the HDL - C was increased （ P 〈 0.05 ）. Conclusion Tonifying - qi and activating - blood decoction has the effects of improving microcirculation, inhibiting cell inflammatory factors and preventing the formation of mictothrombosis on the treatment of early DN, can reduce the D - D level of early DN patients.
