摘  要:目的:探究分析H型高血压(Homocysteine,Hcy≥10μmol/L)、普通高血压和血压正常人群骨密度(Bone mineral density,BMD)情况,讨论H型高血压对BMD的影响。方法:收集城市、郊区、农村50岁以上的男性1100例,其自愿接受腰椎、股骨颈、股骨转子区BMD检查,符合入组标准者900例,受检者血压、血清同型半胱氨酸(Homocysteine,Hcy)进行检测,高血压伴有血清Hcy升高即H型高血压,根据血压检查结果分为H型高血压患者、普通高血压患者、血压正常者,对三类受检者BMD值进行测定,同时统计各组骨量减少、骨质疏松发生率。结果:纳入标准的900例受检者中有698例血压正常者,118例普通高血压患者,84例H型高血压患者,三类受检者高血压病程、体重指数、血压不达标比例比较,差异均有统计学意义(P<0.05);血压正常者腰椎、左侧股骨颈、股骨大转子区域BMD分别为(9.48±0.87)、(7.87±0.93)、(7.48±1.32)g/cm^3,普通高血压患者BMD分别为(8.78±1.03)、(7.23±0.80)、(6.81±0.98)g/cm^3,H型高血压患者BMD为(8.16±1.54)、(6.71±1.02)、(6.28±0.71)g/cm3,三类受检者BMD值比较,差异有统计学意义(P<0.05);血压正常人群骨量正常、骨量减少、骨质疏松比例分别为77.08%、13.75%、9.17%,普通高血压以上比例分别为61.02%、23.73%、15.25%,H型高血压以上比例分别为44.05%、32.14%、23.81%,差异均有统计学意义(P<0.05)。结论:针对中老年男性,高血压和高Hcy对骨密度均有一定的相关性,两者同时存在时,骨密度降低,骨质疏松和骨量减少发生率明显上升。Objective: To explore and analyze the bone mineral density ( BMD ) in patients with H type hypertension (Homoeysteine, Hey≥10μmol/L ), ordinary hypertension and normal blood pressure male, discuss the influence of type H hypertension to BMD.Method: A total of 1100 cases of male in urban, suburban, rural who were more than 50 years old were collected, and 900 cases who conformed to the standard and volunteer received the BMD examination in lumbar spine, femoral neck and trochanterie region were selected as study object, the blood pressure and serum homocysteine ( Homocysteine, Hey ) of subjects were detected.And according to the results of blood pressure, the patients were divided into type H hypertension, ordinary hypertension and normotensive.Then BMD values of three groups were measured, the incidence of osteopenia and osteoporosis were statistically analyzed.Result: 900 cases were enrolled in the study, there were 698 cases with normal blood pressure, 118 cases with hypertension and 84 cases with type H hypertension.There were significant differences in the course of hypertension, body mass index and blood pressure among three groups ( P〈0.05 ) .The lumbar spine, left femoral neck, greater trochanter region BMD in normotensive were ( 9.48±0.87 ), ( 7.87±0.93 ), ( 7.48±1.32 ) g/cm^3 respectively, and BMD of patients in ordinary hypertension were ( 8.78±1.03 ),(7.23±0.80 ), ( 6.81±0.98 ) g/cm^3 respectively, while BMD of type H hypertension were ( 8.16±1.54 ), ( 6.71±1.02 ), (6.28±0.71 ) g/cm^3 respectively, and BMD values of three groups were statistically significant (P〈0.05) .The ratio of bone mass, bone loss and osteoporosis in normotensive were 77.08%, 13.75% and 9.17% respectively, ordinary hypertension were 61.02%, 23.73%, 15.25% respectively, while type H hypertension were 44.05%,32.14% and 23.81% respectively, and the differences were statistically significant among three groups in the determination of bone mass ( P〈0.05 �
