胸腹腔镜联合食管癌根治术与传统三切口手术的临床对比研究

摘　　要：目的:比较胸腹腔镜联合与传统三切口食管癌根治术治疗食管癌的临床效果。方法:回顾分析120例接受胸腹腔镜联合或传统三切口手术的食管癌患者,比较两组的术中情况。术后结果及术后并发症的发生率。结果:两组患者基本情况无显著差异(P>0.05),腔镜组手术时间较长(P<0.05),但术中失血量较开放组少(P<0.05),术后住院时间较开放组短(P<0.05)。腔镜组心肺并发症发生率较开放组更低(P<0.05)。两组淋巴结清扫数量无明显差异(P>0.05)。结论:胸腹腔镜联合食管癌根治术创伤小,并发症发生和术后恢复时间明显优于开放手术,淋巴结清扫数量与开放手术无明显差异,是一种可靠、有效的手术方法,值得临床推广应用。Objective： To compare the efficacy between thoraco laparoscopy and traditional three cuts of esophageal cancer. Methods：Records of 120 esophageal cancer patients who accepted thoraco laparoscopy or open Mckeown esophagectomy were reviewed retrospectively. Intra-operation variables, post-operation outcomes and post-operation complications were compared and analyzed. Results.. The basic conditions were similar between the two groups （P〈0. 05）. Thoraeo laparoscopy group had a longer operating tirne（P〈0. 05）, but less intraoperative blood boss, and shorter postoperative hospital stay （P〈0. 05）. Thoraco laparoscopy group experienced a lower postoperative complication ratio（P 〈0. 05）. There was no statistical difference between the two groups in the number of lymph nodes resected（P〉0. 05）. Conclusion.-Ahhough thoraco laparoscopy Mckeown esophagectomy has a longer operating time,but do less damage to patients, facilitate a more rapid recovery and shorter postoperative hospital stay, and is also capable of satisfied lymph node resection, it prove to be a reliable and efficient surgical procedure.
