急性非等容血液稀释联合控制性降压对老年患者血液保护效应研究

摘　　要：目的探讨急性非等容血液稀释（ANIH）联合控制性降压（cH）对接受外科手术治疗的老年患者血液保护的效果。方法ANIH联合CH组提供术前ANIH联合术中CH，CH组仅提供术中CH处理，ANIH组仅提供术前ANIH。记录3组患者术中失血量、手术时间、不同时间点血液保护指标检测结果变化情况。结果ANIH联合CH组患者术中出血量明显少于CH组、ANIH组，差异均有统计学意义（P〈O．05）；3组患者手术关键步骤结束后血红蛋白（Hb）、血细胞比容（Hct）、血小板计数（Plt）均明显降低，ANIH联合cH组手术关键步骤结束后Hb、Hct、Plt较切皮前变化幅度均明显低于CH组、ANIH组，差异均有统计学意义（P〈O．05）。结论对老年患者实施外科手术时给予ANIH联合CH处理可明显减少术中出血量，有利于提高患者血液保护效果，对保障其手术安全性及生活质量均具有积极意义。Objective To investigate the effect of acute non volume hemodiiution combined with controlled hypotension in the elderly patients undergoing surgical treatment. Methods The study group was given preoperative acute non isometric hemodilution combined with controlled hypotension in the control group, while the control group only received preoperative acute non isometric hemodilution;the blood loss,operation time and different time points of three groups of elderly patients were recorded. Results The hemorrhage group was significantly less than the control group 1 ,control group 2（P〈0.05） ;end key steps after operation of the three groups Hb,Hct,Plt levels were significantly reduced, the end key steps of the surgical research group Hb, Hct and Pit levels before skin incision amplitude was significantly less than the control group 1 and control the 2 group（P〈0.05）. Conclusion The implementation of surgery in elderly patients with acute non normovolemic hemodilution combined with controlled hypotension treatment can significantly reduce the amount of bleeding. It is helpful to improve the protective effect of blood,has the positive sig- nificance to guarantee the operation safety and quality of life.
