摘　　要：目的研究社区健康服务中心的慢性病管理效果并总结管理经验。方法回顾性分析北京市平谷区平谷镇社区2014年1月至2016年12月慢性病管理的相关资料，通过对存档患者血压、血糖及治疗依从性等方面数据的统计，与管理初期的数据进行对比，评价对慢性病管理的效果。结果管理近期患者吸烟、饮酒、食盐过多等不良生活方式发生率均明显少于管理初期，患者血压值（收缩压／舒张压）明显低于管理初期，空腹血糖、餐后2h血糖、糖化血红蛋白水平均明显低于管理初期，差异均有统计学意义（P〈O．05）。结论社区健康服务中心应用慢性病管理方案后取得了满意成效，对社区慢性病的防治具有十分重要的作用。Objective To study the effect of chronic disease management in community health service center and summarize the management experience. Methods A retrospective analysis of information of chronic disease management in Pinggu community from January 2014 to December 2016 was conducted; we collected the statistical data on blood pressure, blood glucose and the com- munity archive treatment compliance and other aspects of the management, and then compared the datas. All the datas were com- pared with the initial data,and the the effectiveness of chronic disease management in community was evaluated. Results compared with the initial management,the smoking, drinking, excessive salt and other unhealthy habits were significantly lower, blood pres- sure, fasting plasma glucose, postprandial 2 hour blood sugar and glycated hemoglobin levels were all significantly lower （P〈0. 05）. Conclusion the application of chronic disease management program in the community health service center achieved satisfacto- ry results, and played very important role in the prevention and treatment of chronic disease.
