星状神经节联合椎旁神经阻滞治疗交感型颈椎病疗效评估

摘　　要：目的评价星状神经节阻滞（SGB）治疗、SGB联合椎旁神经阻滞（pCPB）治疗交感型颈椎病的临床疗效。方法将交感型颈椎病患者80例随机分为A组和B组，每组40例。A组给予SGB治疗，B组给予SGB联合pCPB治疗，两组均进行SGB治疗，每天1次，15次为1个疗程，治疗1个疗程（15d）。B组同时进行pCPB治疗，以曲安奈得、利多卡因、维生素B12、维生素B1与生理盐水混合进行pCPB治疗，每周1次，4次为1个疗程。观察两组患者症状、体征变化，比较两组患者的疗效。结果治疗1个疗程后患者临床症状、体征均明显好转，B组明显优于A组，B组优良率、总有效率均明显高于A组，差异均有统计学意义（P〈O．05）。结论SGB、SGB联合pCPB治疗交感型颈椎病均具有明显临床效果，后者的近期效果更佳。Objective To evaluate the clinical efficacy of stellate ganglion block and stellate ganglion block＋ paravertebral nerve block in treating c sympathetic cervical spondylosis. Methods 80 adult patients with sympathetic cervical spondylosis were random- ly divided into two equal groups of 40 patients in each. The patients were treated （SGB） in group A and stellate ganglion btock-~pa- ravertebral nerve block（SGB＋PNB） in group B. Stellate ganglion block was carried out for once a day and one course of treatment for fifteen times. In the meantime, sacral block with paravertebral nerve block was carried out for once a week and one course of treatment had four administrations, in group B. The analgesic composed of triamcinolone acetonide, lidocaine, vitamin B12, vitamin B1 and normal saline was injected into paravertebral nerve. The cases in the two groups were treated wtith one course of treatment （fifteen days）. The changes in according to the symptom and sign scoring criteria were observed and compared before treatment. The therapy efficacy was compared to between the groups. Results After one course of treatment,the symptom and sign were sig nificantly improved in the two groups（P〈0.05）. It was better in group B than in group A（P〈0.05）. The total fine rate was 70. 0% （28/40 cases）and 92.5% （37/40 cases）, the difference was statistical significant（P〈0.05）. The total effective rate was 75.0 （32/40 cases）and 97.5 % （39/40 cases）respectively in group A and group B after treatment, the difference was statistical significant （P〈0.05）. Conclusion The stellate ganglion block and stellate ganglion block＋paravertebral nerve block are all effective approa- ches in the treatment with symoathetic cervical spondylosis. The latter maybe be superior to the former.
