摘  要:目的评价CBA检测经特异抗原刺激后PBMC上清液中细胞因子浓度水平在菌阴结核病中的诊断价值。方法将研究对象分为健康人群、肺炎患者、潜伏感染者和菌阴结核病患者四组,运用CBA技术对PBMC刺激培养后的上清液中IL-2、IL-4、IL-6、IL-10、TNF-a、IFN-g和IL-17的浓度进行检测,并对检测结果统计分析。结果菌阴结核病患者的IL-2、IL-6、IL-10、TNF-a、IFN-g分泌水平均高于健康人群和肺炎患者组,差异有统计学意义;IL-4和IL-17无差异。菌阴结核病患者组的IL-2低于潜伏感染组,差异有统计学意义;TNF-a、IFN-g均高于潜伏感染组,差异有统计学意义;IL-4、IL-6、IL-10、IL-17结果差异无统计学意义。结论 CBA对PBMC刺激后上清液的细胞因子检测结果能为菌阴结核病的诊断提供有效的依据,对于控制结核病疫情的发展,并实现最终消灭结核病的长远目标具有极其重要的意义。Objective To evaluate the value of CBA technology in smear negative tuberculosis diagnosis by detecting PBMC supernatants cytokine levels which stimulation by specific antigen. Methods The subjects were divided into healthy people,pneumonia patients,tuberculosis latent infection and smear-negative TB patients. To detected IL-2,IL- 4, IL-6, IL-10, TNF-a, IFN-g and IL-17 in PBMC culture supernatants after specific antigen stimulation by CBA tech- nology,then statistical analysis the test results. Results The IL-2,IL-6,IL-10,TNF-a and IFN-g secretion levels of smear-negative TB patients were higher than those of both healthy people and pneumonia patients, the difference had statistically significant. The concentration of IL-4 and IL-17 were no difference. The concentration of IL-2 of smear-neg- ative TB patients was lower than latent infection tuberculosis,the difference had statistically significant. The concentra- tion of TNF-a and IFN-g were higher than latent infection tuberculosis, the difference had statistically significant. The concentration of IL-4,IL-6,IL-10 and IL-17 were no difference. Conclusion The results of cytokine test in PBMC su- pernatants after stimulation could provide the diagnosis of smear-negative TB patients an effective basis,had great sig- nificance to control the development of TB epidemic and achieve the eventually goals of eliminate TB.
