国内首例植入心脏起搏器后留置导引钢丝患者的救治与围手术期护理

摘　　要：目的探讨植入脏起搏器后留置导引钢丝患者的救治与围手术期护理。方法 2016年10月四川大学华西医院收治1例心脏严重增大的患者,因病情原因,无法使用传统起搏器安置法,现采用植入永久性心脏起搏器后导引钢丝保留体内不撤出,起支架和固定作用,以防电极脱位的创新救治方法,并对其实施个性化的围手术期护理。结果患者手术成功,愈后良好,对护理质量满意度高,术后随访期间无不适。结论以此该方式完成心脏起搏器植入术尚属国内首创。针对临床特殊患者,采用植入起搏器后保留导丝的创新救治方法具有可行性,围手术期的个性化护理是保证治疗效果的关键。Objective To explore the innovative treatment and perioperative nursing of a patient undergoing implantation of permanent cardiac pacemaker with retained guide wire.Methods A patient with severe cardiac dilatation was admitted into West China Hospital on October of 2016.In view of the extremely enlarged heart,the regular implantation of pacemaker proved to fail on this patient,so the patient was treated with the innovative treatment of implanting permanent cardiac pacemaker with retained guide wire which could hold and fix the electrode in case of mobilization.Meanwhile,the individualized perioperative nursing care was implemented.Results The patient′s operation was successful with favorable prognosis.The quality of nursing care was highly regarded by the patient and no adverse reaction was reported during the follow-up.Conclusion The operation method of implanting permanent cardiac pacemaker with retained guide wire is adopted innovatively in China,and it is feasible for patients with special clinic conditions.Meanwhile,the implementation of individualized perioperative nursing is the key to the positive therapeutic effect.
