摘　　要：目的持续跟踪和评价高血压管理分阶段培训的长期效果,探索适宜的慢性病管理培训模式。方法对南京市社区医生进行高血压规范化管理的两个阶段培训,并长期进行效果评价。使用Epidata录入数据,运用SPSS 22软件进行数据整理和统计分析。结果 2013年第一阶段基础培训时,调查发现社区医生在"诊断鉴别(继发性)高血压"、"控制(治疗)目标及方案"和"高血压转诊标准"方面得分较低,经过培训,得分大幅度提高。2年后跟踪评价发现,第一阶段培训对"控制(治疗)目标及方案"知识的掌握仍存在积极的效果,但"诊断鉴别(继发性)高血压"、"高血压转诊标准"的知识点掌握水平又恢复到第一阶段培训前;第二阶段培训时发现,接受过基础培训的学员培训效果更加显著。结论基础培训对于慢性病管理知识和技能的提高有积极的意义。循序渐进的分阶段培训有助于慢性病管理知识的加强和巩固,社区医生通过"培训-实践-再培训-再实践"的螺旋上升过程,不断提高了技能水平、增强了自信心。OBJECTIVE To evaluate the long-term effect of the hypertension management phased training, and provide a standardizedtraining model for chronic diseases management. METHODS Held two stage training of chronic disease standardized management forcommunity doctors in Nanjing, and then carried out questionnaire survey, using Epidata to input data, and SPSS 22 for data process-ing and statistical analysis. RESULTS The first phase training in 2013, ＂differential diagnosis of secondary hypertension＂, ＂goal andplan of controlling and treatment ＂ and ＂referral standard＂ got lower scores, and after the training score greatly raised. 2 years later,the first phase training still existed positive effect, however, the master of the orther two knowledges returned to the levels befor thetraining of 2013. But, compared to who did not participate in the first phase training, the trainee had more significant progress duringthe second phase of training in 2015. CONCLUSION The basic training had a positive significance for the improvement of chronic dis-ease management. Income of basic and practical knowledge is obvious. Continuous training onld help to consolidate the knowledge,and provide spiral rising process of training-practice-retraining-practice for community doctors to improve skills and enhance confi-dence.
