北京地区2013—2016年医保参保者首诊基层情况分析

摘　　要：利用2013—2016年北京地区1 980份实证调查数据,分析北京市参保者首诊选择在基层医院的情况。在被调查中选择去市级医院首诊的患者逐渐减少,选择首诊区级医院的患者增多;享有城镇职工医保的患者选择首诊在基层医院的比例最高;被调查者对于基层医院的医疗水平和非基层医院的收费水平感到不满意。北京地区分级诊疗制度初见成效,在政策进一步实施过程中可尝试探索调整三类医保在基层医疗机构的就医报销比例。同时,基层医疗机构技术的提升应先行于分级诊疗制度的实施。1980 empirical survey data were used to analyze of the patients＇ choices of basic initial diagnosis in the Beijing. It foundthat the patients who chose to go to the municipal hospital for the first time diagnosis were reduced gradually, and who chose to go tothe district hospital for the first time diagnosis were increased. It also found that more patients who enjoy urban employee basic medi-cal insurance chose to go to the primary hospital for the first time diagnosis than people who enjoy other basic medical insurance. Therespondents were dissatisfied with the level of medical treatment and the level of non-primary hospital charges. It was believed thatgrading diagnosis and treatment have achieved initial success in Beijing. In the future, the government should try to adjust the threebasic medical insurance reimbursements in primary hospital. At the same time, the promotion of primary medical technology shouldbe advance.
