通元针法联合药物治疗顽固性多囊卵巢综合征不孕临床观察

摘　　要：目的观察通元针法联合来曲唑、HCG治疗顽固性多囊卵巢综合征(PCOS)不孕的临床疗效。方法选取对克罗米芬(CC)促排卵抵抗的顽固性PCOS不孕患者60例,随机分为两组,治疗组采用通元针法联合来曲唑、HCG治疗,对照组采用来曲唑、HCG治疗,疗程为3个月。治疗前及治疗后监测患者血清雌二醇(E2)、促卵泡生成激(FSH)、促黄体生成激素(LH)、睾酮(T);子宫内膜情况、排卵率及妊娠率。结果 3个月经周期后治疗组和对照组的FSH均明显升高(P<0.05),LH明显降低(P<0.05),LH/FSH比值显著下降(P<0.05),T水平也明显下降(P<0.05),治疗组和对照组患者治疗后E2水平较治疗前下降(P<0.05),且治疗组下降更为显著(P<0.05)。治疗组排卵率、妊娠率较对照组高(P<0.05)。结论通元针法联合来曲唑、HCG能明显降低患者的LH、E2及T的水平,提高FSH水平,有效改善顽固性PCOS患者的生殖内环境,提高排卵率和妊娠率。Objective To observe the clinical efficacy of Tong Yuan （short for Tong Du Tiao Shen, Yin Qi Gui Yuan, referring to unblocking the Governor Vessel, regulating mind, and guiding qi back to the origin） needling method plus Letrozol and human chorionic gonadotropin （HCG） in treating refractory sterility caused by polycystic ovary syndrome （PCOS）. Method Sixty clomiphene citrate-resistant PCOS-induced sterility patients were recruited and randomized into two groups. The treatment group was intervened by Tong Yuan needling plus Letrozol and HCG, while the control group was intervened only by Letrozol and HCG, both for 3 months. Prior to and aider the treatment, the levels of estradiol （E2）, follicle stimulating hormone （FSH）, luteinizing hormone （LH）, and testosterone （T） were detected; the endometrium, ovulation and pregnancy rates were also observed. Result After 3 menstrual cycles, the level of FSH increased significantly （P〈0.05）, LH decreased significantly （P〈0.05）, LH/FSH declined significantly （P〈0.05）, and the level of T also dropped significantly （P^0.05） in both treatment group and control group; the level of E2 also dropped significantly after the treatment in both groups （P〈0.05）, and the decrease in the treatment group was more significantly than that in the control group （P〈0.05）. The ovulation and pregnancy rates in the treatment group were significantly higher than those in the control group （P〈0.05）. Conclusion Tong Yuan needling plus Letrozol and HCG can significantly down-regulate the levels of LH, E2 and T, up-regulate the level of FSH, effectively improve the internal reproductive environment of refractory PCOS patients, and enhance the ovulation and pregnancy rates.
