腹腔镜腹股沟疝修补术治疗老年腹股沟疝的临床疗效

摘　　要：目的探讨老年腹股沟疝采用腹腔镜腹股沟疝修补术治疗的疗效。方法方便选取68例于2013年6月—2015年6月期间该院接收的老年腹股沟疝患者,将入选者随机分为对照组(采取无张力修补术治疗,n=34)与观察组(采取完全腹膜外腹腔镜疝修补术,n=34),观察两组手术情况。结果观察组患者术程较对照组显著要长(P<0.05),分别为(57.3±6.8)min、(47.5±6.9)min,在创口直径、术中失血量、术后止痛药使用次数、住院时间、疼痛评分上,观察组分别为(10.5±1.8)mm、(20.5±4.3)m L、(0.6±0.2)次、(3.2±0.8)d、(3.6±1.2)分,相比于对照组均显著要低,分别为(86.5±5.7)mm、(34.6±4.5)m L、(1.6±0.4)次、(6.5±0.7)d、(6.7±2.1)分,(P<0.05);观察组患者复发率2.9%显著低于对照组的17.6%,且观察组切口感染、慢性疼痛、异物感分别为5.9%、2.9%、2.9%,均显著低于对照组,分别为26.5%、23.5%、26.5%,(P<0.05)。结论老年腹股沟疝采取腹腔镜腹股沟疝修补术治疗,创伤小,术后恢复快,且并发症及复发率低,安全可行,具有推广价值。Objective To study the curative effect of laparoscopic inguinal hernioplasty in treatment of senile inguinal region hernia. Methods 68 cases of senile patients with inguinal region hernia treated in our hospital from June 2013 to June2015 were convenient selected and randomly divided into two groups with 34 cases in each, the control group adopted the tension-free hernia repair, while the observation group adopted the totally extraperitoneal laparoscopic hernioplasty, and the operation situation of the two groups was observed. Results The operation course in the observation group was obviously longer than that in the control group（P〈0.05）[（57.3±6.8）min vs（47.5±6.9）min], and the wound diameter, intraoperative bleeding amount, postoperative painkiller use frequency, length of stay and pain scores in the observation group were obviously lower than those in the control group [（10.5 ±1.8）mm,（20.5 ±4.3）m L,（0.6 ±0.2）times,（3.2 ±0.8）d,（3.6 ±1.2）points vs（86.5±5.7）mm,（34.6±4.5）m L,（1.6±0.4）times,（6.5±0.7）d,（6.7±2.1）points]（P〈0.05）, and the recurrence rate in the observation group was obviously lower than that in the control group,（2.9% vs 17.6%）, and the incidence rate of incision infection,chronic pains and foreign body sensation in the observation group were obviously lower than those in the control group（5.9%,2.9%,2.9% vs 26.5%,23.5%,26.5%）,（P〈0.05）. Conclusion The wound of laparoscopic inguinal hernioplasty in treatment of senile inguinal region hernia is small with rapid recovery after operation, and the incidence rate of complications and recurrence rate are low, and it is safe and feasible, which is worth promotion.
