摘　　要：目的探讨同型半胱氨酸(Hcy)对老年高血压病人认知功能的影响。方法将127例老年高血压病人根据血Hcy水平分为2组:H型高血压组(Hcy≥10μmol/L)80例,非H型高血压组(Hcy<10μmol/L)47例,分别测定2组空腹血糖、甘油三酯、总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、脂蛋白-a、肌酐、尿酸、丙氨酸氨基转移酶、天冬氨酸氨基转移酶、超敏C反应蛋白、24 h平均血压,所有病人进行Mo CA量表评分。结果老年H型高血压病人较非H型高血压病人更易出现认知功能障碍(P<0.01),回归分析显示血Hcy(P<0.01)、年龄(P<0.01)是老年高血压病人认知功能障碍的独立危险因素。结论老年人H型高血压病人认知功能水平下降,老年高血压病人中血Hcy水平是认知功能障碍的独立危险因素之一。Objective To investigate hypertension, and to investigate the effects of the cognitive function of the elderly patients with H type homocysteine （Hcy） on the cognitive function. Methods 127 elderly patients with hypertension were divided into two groups according to blood Hcy levels, H type hypertension group （n= 80, with a level of Hcy≥ 10 μmol/L） and non-H type hypertension group （n= 47, with a level of Hcy 〈 10 μmol/L）. The levels of fasting plasma glucose, triglyceride, total cholesterol, low density lipoprotein cholesterol, high density lipoprotein cholesterol, lipoprotein a, creatinine, uric acid, alanine transaminase, aspartate aminotransferase, high-sensitivity C reactive protein （hs-CRP） and blood pressure were examined at the same time. All patients were assessed by Montreal cognitive assessment（MoCA） to screen cognitive impairment. Results Elderly patients with H type hypertension were more likely to present with cognitive impairment（P〈0. 01 ）.Logistic regression analysis showed that blood Hcy level （ P〈 0. 01 ）, age （ P〈 0. 01 ） were independent risk factors of cognitive impairment in elderly patients with hypertension. Conclusions H type hypertension can influence the cognitive function in elderly.The level of blood Hcy is an independent risk factor of cognitive impairment in elderly patients with hypertension.
