喜炎平与阿维A胶囊联合治疗斑块型银屑病的疗效及其对趋化因子RANTES及MCP-1的影响

摘　　要：目的 探讨喜炎平与阿维A胶囊联合治疗斑块型银屑病的疗效及其对分泌活化调节蛋白（RANTES）、单核细胞趋化蛋白-1（MCP-1）水平的影响.方法 抽取2015年1月至2016年4月84例斑块型银屑病患者,根据治疗方案不同分为对照组（n=42）和研究组（n=42）.对照组仅采用阿维A胶囊治疗,研究组联合采用阿维A胶囊与喜炎平治疗.对比治疗前后两组银屑病皮损面积和严重程度指数（PASI）、肿瘤坏死因子-α（TNF-α）、RANTES、MCP-1水平变化情况,并统计两组临床疗效、不良反应发生率.结果 治疗前两组PASI评分及TNF-α水平比较差异未见统计学意义（P〉0.05）,经治疗,研究组PASI评分[（4.52±0.69）分]、TNF-α水平[（6.55±1.01）pg/ml]明显优于对照组[（8.32±1.77）分、（12.50±2.46）pg/ml],差异有统计学意义（P〈0.05）;研究组总有效率（88.10%）明显高于对照组（69.05%）,差异有统计学意义（P〈0.05）;治疗前两组RANTES、MCP-1水平比较差异未见统计学意义（P〉0.05）,经治疗,研究组RANTES[（21.75±6.34）]pg/ml、MCP-1[（140.08±23.57）]pg/ml明显优于对照组[（35.81±6.05）pg/ml、（152.28±15.71）pg/ml],差异有统计学意义（P〈0.05）;研究组不良反应总发生率（14.28%）与对照组（11.90%）比较差异未见统计学意义（P〉0.05）.结论 联合采用喜炎平与阿维A胶囊治疗斑块型银屑病疗效确切,可有效缓解患者临床症状,降低炎症因子及RANTES、MCP-1表达水平,提高治疗效果,且具有较高安全性,值得推广应用.Objective To investigate the curative effect of Xiyanping injection combined with avermectin A capsules on plaque psoriasis and its influence on activation regulated protein （RANTES）, monocyte chemoattractant protein-1 （MCP-1） level.Methods Eighty-four patients with plaque psoriasis were selected from January 2015 to April 2016, according to different treatment options, they were divided into control group （n=42） and study group （n=42）.The control group was only treated with avermectin A capsule, the study group was used avermectin A capsule and Xiyanping injection.The psoriasis area and severity index （PASI）, tumor necrosis factor alpha （TNF-α）, RANTES, MCP-1 level changes between the two groups were compared before and after treatment, and the clinical curative effects and adverse reaction rates were analyzed between the two groups.Results Before treatment, the PASI score and TNF-α levels showed no significant difference between the two groups （P〉0.05）.After treatment, the PASI score （4.52±0.69）, levels of TNF-α [（6.55±1.01） pg/ml] in study group were significantly better than those in the control group [（8.32±1.77）, （12.50±2.46） pg/ml], the differences were significant （P〈0.05）.The total effective rate of the study group （88.10%） was significantly higher than that of the control group （69.05%）, and the difference was significant （P〈 0.05）;Both groups of RANTES and MCP-1 levels compared with before treatment had no significant difference （P〉0.05）.After the treatment, the RANTES[（21.75±6.34） pg/ml], MCP-1[（140.08±23.57） pg/ml] in study group were significantly better than those in the control group [（35.81±6.05） pg/ml, （152.28±15.71） pg/ml], the differences were significant （P〈 0.05）;The incidence rate of adverse reactions in the study group （14.28%） had no significant difference compared with the control group （11.90%, P〉0.05）.Conclusions The curative effect of Xiyanping injection combined with ave
