中西医结合治疗干燥综合征肾损害的临床疗效观察

摘　　要：目的观察我科自研中药汤剂(曾用名"肾衰竭1号")联合中等剂量泼尼松治疗原发性干燥综合征(p SS)并肾间质损害的临床疗效和安全性。方法就诊于泰安市中心医院肾内一科门诊或病房、确诊为原发性干燥综合征并间质肾损害的患者,将他们随机分为对照和治疗两组,对照组给予对症及泼尼松、治疗组在此基础上给予我科自研中药汤剂1剂qd,连用4周。观察两组患者临床症状、体征、某些化验指标的变化。结果总疗效:治疗组显效17例、有效13例、无效3例,总有效率为90.91%;对照组显效13例、有效14例、无效6例,总有效率为81.82%,χ~2检验,差异有统计学意义(P<0.05);治疗组和对照组尿素氮分别下降(6.84±1.29)mmol/L和(3.21±0.87)mmol/L(t=2.733,P=0.01),肌酐分别下降(91.34±15.14)μmol/L和(65.79±12.21)μmol/L(t=2.054,P=0.004),t检验,差异均具有统计学意义(均P<0.05);治疗组出现胃肠道不适及恶心、呕吐1例,转氨酶升高1例。结论和对照组相比,我科自研中药汤剂可显著改善原发性干燥综合征(p SS)并肾间质损害患者的中医症候群,有效降低尿素氮、肌酐水平,且患者不良反应轻微、耐受性好。Objective： To study the clinical effect and adverse reaction of combining traditional Chinese and western medicine on the patients of Primary sjogren＇s syndrome with renal lesion. Methods： Patients diagnosed with primary sjogren syndrome and interstitial renal lesion were randomly divided into two groups, control group was given prednisone and treatment, on the basis, developed Chinese herbal decoction for the treatment group, the variation between before and after treatment were observed in the clinical symptoms, signs and some laboratory indexes of patients. Results ： After 4 weeks of treatment, The overall effective rate was higher in the treatment group than in the control group（90.91% vs 81.82% ,PP 〈 0. 05 ） ~ The treatment groups lower average value in urea nitrogen is （6. 84 ± 1.29 ）mmol/L, the average reduced value of ereatinine is（91.34 ± 15.14）Ixmol/L, and there was significant difference compared to the control group（ P 〈 0. 05 ） ; one patient of reatment group appeared gastrointestinal discomfort, one patient＇s transaminase went up in reatment group. Conclusions： compared with control group, Chinese herbal decoction may be effective in improving clinical symptoms and laboratory indicators of primary Siogren＇s syndrome, and the side effects of patients was mild.
