HRCT多平面重组矢状位在LVAS诊断中的应用

摘　　要：目的运用HRCT多平面重组矢状面图像,对前庭导水管径线进行测量,探讨在诊断LVAS中的应用。方法搜集正常人80例,对颞骨进行HRCT扫描,将所得图像行MPR后处理,重组两侧前庭导水管矢状位图像,测量前庭导水管在此图像上中段的宽度,并建立95%的医学参考值范围。另搜集16例临床表现为听力异常,颞骨HRCT扫描后,横轴位测量初步诊断为LVAS患者,与正常观察对象进行统计学分析。结果 MPR重建出的矢状位图像对前庭导水管的显示率高于直接轴位图像扫描显示率,并且在左右两侧及男女性别无差异统计学意义。正常组导水管中段宽度值在重组矢状位方向上95%的医学参考值范围:左侧0.4894~0.9790 mm;右侧0.5190~1.0278 mm。16例临床诊断为听力异常患者,横轴位测量初步诊断为LVAS患者,重组矢状位中段值范围1.2~3.9 mm,与正常组比较有统计学差异。结论前庭导水管在矢状位重建上中间宽度值≥1.1 cm,诊断为前庭导水管扩大。同时伴有临床听力减低及相应病史,诊断为大前庭导水管综合征。Objective： To investigate the application in diagnosis of LVAS in the use of HRCT multi planar reconstruction sagittal images of vestibular aqueduct. Methods：In the collection of 80 cases of normal controls, HRCT scan of the temporal bone, the resulting image for MPR post processing, restructuring on both sides of the vestibular aqueduct sagittal images, measurement of vestibular aqueduct in this image in the middle of the width, and the establishment of 95% medical reference range were done. In the collection of 16 cases of abnormal heating, temporal bone HRCT scan were done after initial diagnosis of axial measurement for LVAS patients and were statistically compared with those of normal subjects. Results： The 95% of the medical reference range in the middle of aqueduct width reorganization sagittal direction in the normal group was： left 0. 4894 -0. 9790 mm, and tight 0. 5190 - 1. 0278 mm. In the 16 cases of clinically diagnosed patients with abnormal hearing, the initial diagnosis of axial measurement for patients with LVAS, the recombinant sagittal middle range from 1.2 - 3.9 mm, there was significant difference compared with the normal group. Conclusion： Vestibular aqueduct in sagittal reconstruction on the intermediate width value is greater than or equal to 1.1 cm, to expand the diagnosis of vestibular aqueduct. At the same time with heating impairment and the corresponding clinical history, the diagnosis of large vestibular aqueduct syndrome can be made.
