摘  要:目的探讨髓母细胞瘤的常规MRI表现及DWI、T_2FLAIR信号特点,提高MRI对髓母细胞瘤的诊断水平。方法回顾性分析经病理证实的18例髓母细胞瘤患者,对肿瘤的常规MRI平扫、增强、DWI及T_2FLAIR信号特点进行分析。结果 18例髓母细胞瘤位于小脑蚓部14例,小脑半球4例;病变呈实质性10例,囊实性8例;沿孔生长6例;前方有脑脊液环绕10例。增强扫描轻中度强化6例,明显强化10例,2例呈显著强化。18例病变DWI均呈高信号;T_2FLAIR呈等信号14例,高信号4例。结论髓母细胞瘤的常规MRI检查会导致部分病变误诊,结合DWI及T_2FLAIR可明显提高影像诊断的准确率。Objective To analyze the routine MRI and DWI( Diffution Weighted Imaging),T_2FLAIR( T_2Fluid-attenuated Inversion Recovery) feature of medulloblastoma,in order to optimize diagnostic accuracy of medulloblastoma. Methods 18 cases of medulloblastoma confirmed by histopathology were analyzed retrospectively on routine MRI,DWIand T_2FLAIR findings. Results Among 18 cases with medulloblastoma,14 lesions located at the cerebellar vermis,and the other 4 cases were in the cerebellar hemisphere; medulloblastomas showed as solid tumors in 10 patients,cyst-solid masses in 8 patients; 6 cases growed along the hole; ahead of cerebrospinal fluid in 10 cases. The solid components presented with subtle or medium enhancement in 6 cases,marked enhancement in 10 cases,and significantly enhancement in 2 cases. All of the 18 cases were with hyper-intense signal on DWI; 14 cases were with isointense signal and 4 cases with hyper-intense signal on T_2FLAIR. Conclusion The routine MR imaging of medulloblastoma can cause some cases misdiagnosis,combination of DWI and T_2FLAIR may help to obviously improve imaging diagnostic accuracy.
