紫外线联合中医辨证对掌跖脓疱病外周血T细胞亚群的影响

摘　　要：目的观察中药泡洗联合窄谱紫外线治疗掌跖脓疱病的临床疗效及对外周血T淋巴细胞亚群的影响。方法 64例掌跖脓疱病患者随机分为2组,中药组单纯外用自制中药治疗,联合组用中药泡洗联合紫外线治疗,治疗4周为1个疗程,连续治疗8周,治疗4周、8周后分别判定疗效。治疗前后及时检测外周血T淋巴细胞表型中CD_3^+,CD_4^+,CD_8^+细胞的表达情况及判定疗效。结果治疗4周后,中药组有效率为53.13%,联合组有效率为71.88%,2组比较差异有统计学意义(P<0.05);治疗8周后,中药组有效率为68.75%;联合组有效率为90.63%,2组比较差异有统计学意义(P<0.05)。治疗8周后联合组CD_4^+细胞、CD_4^+/CD_8^+细胞比值均明显高于中药组,差异有统计学意义(P<0.05)。结论中药泡洗联合紫外线治疗掌跖脓疱病的临床疗效明显,且不良反应少,安全性高。Objective To observe the therapeutic effects of traditional Chinese medicine（ TCM） soaking and washing combined with narrow spectrum ultraviolet on palmoplantar pustulosis and their effects on T lymphocyte subsets in peripheral blood of patients with palmoplantar pustulosis. Methods Sixty-four patients with palmoplantar pustulosis were randomly divided into two groups： TCM group and combination treatment group. The patients in TCM group were treated only by self-made external use TCM,however,the patients in combination treatment group,on the basis of TCM group,were treated by narrow spectrum ultraviolet,with a treatment course of 4 weeks for 2 treatment courses in both groups. The therapeutic effects and the expression levels of CD3＋,CD4＋,CD8＋were detected on 4w,8w after treatment for both groups. Results After 4-week treatment,the effective rate in TCM group and combination treatment group was 53. 13%,71. 88%,respectively,there was a significant differnce between two groups（ P〈 0. 05）. After 8-week treatment, the effective rate in TCM group and combination treatment group was 68. 75%,90. 63%,respectively,there was a significant differnce between two groups（ P 〈0. 05）. After 8-week treatment,the proportion of CD4＋cells,CD4＋/CD8~＋cells in combination treatment group was significantly higher than that in TCM group（ P〈 0. 05）. Conclusion The TCM soaking and washing combined with narrow spectrum ultraviolet has obvious therapeutic effects on palmoplantar pustulosis,with better safety and few adverse reactions.
